Introduction : Paroxysmal nocturnal hemoglobinuria (PNH), characterized by an increased sensitivity of erythrocytes to the hemolytic action of complement, is an uncommon hematologic disease. The most common neurologic complication of the PNH is a cerebral venous thrombosis. However, arterial thrombosis associated with PNH is rare and has not been reported in Korea. Case Report : A 61-years-old woman presented with drowsy mental status and right hemiplegia, preceded by intermittent dark urination and malaise. The diagnosis of PNH was based on positive sucrose hemolysis test and HAM test. Brain MRI revealed left anterior cerebral artery territory infarction. After medical treatments including blood transfusion, steroid and antiplatelet agent, her hematologic and clinical findings improved. Conclusion : PNH may play a role in development of cerebral arterial thrombosis. Korean Journal of Stroke 2000;2(1): 117~120
