Background : Sensory dysfunction in lateral medullary infarctions may manifest diversely according to the location of the lesion. However, ipsilateral sensory symptoms in the limbs are rare and have not been well recognized. Case Report : We described a 46-year-old man who presented with paresthesia in the left arm which was followed by typical lateral medullary syndrome: vertigo, gait ataxia, Horner sign and contralateral impairment of the spinothalamic sensory modalities. MRI showed a small infarct in the left lower medulla extending to the dorsomedial area. The ipsilateral sensory symptoms in the arm may be explained by an involvement of either the large myelinated fibers at the nucleus cuneatus or dicussating lemniscal fibers. Conclusion : A sensory symptom in the ipsilateral limbs is a rare but distinct variant of lateral medullary infarction.
