Background : Decompressive hemicraniectomy may be a useful lifesaving therapy in large hemispheric infarction, though there are only a few experimental data. The aim of this study to investigated the efficacy of decompressive hemicraniectomy in a rat model of focal cerebral ischemia. Methods : Permanent focal cerebral ischemia was induced in 81 male Sprague-Dawley rats (250 ∼350g) by occluding the left MCA using intraluminal thread. Rats were divided into 2 groups; control group (CG, n=51) with ischemia only and hemicraniectomy group (HG, n=30) with decompressive hemicraniectomy at 6 hours after ischemia. Then, rats in each group were further divided into 2 groups according to the duration of ischemia; CG3 (n=29) and HG3 (n=21) for 3 days, and CG7 (n=22) and HG7 (n=9) for 7 days. For hemicraniectomy, a bone flap (10 x 10 mm) was created in the left temporal area and the dura was opened by incision. The volume of infarction was measured using image analyzer after staining with triphenyltetrazolium chloride. Results : The infarction(mean ±SD, mm 3) was significantly decreased by hemicraniectomy in rats with 7 days of ischemia(122.4 ±40.2 in HG7 vs 191.6 ±42.4 in CG7, p<0.01) but not in rats with 3 days of ischemia(236.3 ±94.3 in HG3 vs 260.2 ±100.8 in CG3, p>0.1). The mortality showed a trend to decrease in rats with hemicraniectomy without statistical significance(44.8% vs 28.6% in CG3 and HG3, 50% vs 11.1% in CG7 and HG7, p>0.1). Conclusion s : Our results showed that the decompressive hemicraniectomy in animal model of permanent MCA occlusion was beneficial by decreasing the size of infarction.
