Background and Objectives : We aimed to define diurnal variations of stroke onset timing in Hallym Stroke Registry Data Bank(HSRDB) and analyze demographic and clinical factors related to circadian rhythm. Methods : Five hundred and twenty consecutive acute ischemic stroke patients were recruited between January 1999 and December 2000. The onset time was identifiable in 365 patients(70%) and divided in quartiles(0:00 to 5:59, 6:00 to 11:59, 12:00 to 17:59, 18:00 to 23:59). Age, gender, temporal profile, lesion location, stroke risk factors and etiologic stroke subtypes were investigated. Results : Among 365 patients, 93 strokes(25.4%) occurred during the first quartile, 98 strokes during the second quartile, 98 during the third quartile and 76 during the fourth quartile(p=0.310). The occurrence of stroke in women was more prevalent during the first and second quartile than men(30.3%, 29.6% in women, 22.1%, 24.9% in men, p<0.01). Transient ischemic attacks(TIA) occurred more frequently during the third quartile(p<0.01). The lesion locations, risk factors and age were not different according to the time periods. Among the etiologic stroke subtypes, only cardioembolism showed diurnal variation pattern and occurred frequently during the second quartile(p<0.01). Conclusion : Even though there was no significantly variable circadian pattern of stroke onset time in HSRDB, ischemic stroke occurred more frequently during early morning and afternoon. Gender, TIA and cardioembolism are factors related to the diurnal variation. Korean Journal of Stroke 2002;4(2):65~71
