Department of Neurology, College of medicine, Inje University, Sanggye Paik Hospital Spinal cord infarction accounts for 1% of all cases of stroke. Motor weakness, sensory disturbances and autonomic dysfunction are typical clinical features of spinal cord infarction. A chest pain as a presenting symptom of spinal cord infarction is unusual. A previously healthy 42-year old woman presented with an acute chest pain resembling angina pectoris, which preceded the development of paraplegia and sensory disturbances. Evaluation for cardiac diseases was normal. MRI of the spinal cord disclosed high signal intensity on T2 weighted image between C7 and T2 spinal cord levels. This segmental level coincides with the convergence of ascending afferent tracts from cardiac plexus. Acute chest pain may be the initial presenting symptom of spinal cord infarction.
