Department of Neurology, College of Medicine, Seoul National University*Department of Neurology, Eulji University School of Medicine† Pulmonary arteriovenous malformation (AVM) is a rare cause of stroke via paradoxical embolism. We report a41-year-old woman with cerebral embolic infarct with occult pulmonary AVM, who presented with visual fielddefect of sudden onset. She has never experienced dyspnea, bleeding tendency or skin lesion, except migrainousheadache. Transesophageal echocardiography and transcranial doppler (TCD) revealed right-to-left shunt. TCDwith saline contrast medium showed multiple microembolic signals not only during Valsalva maneuver but alsoduring normal breathing. Several pulmonary AVMs were detected on chest CT, which were emboilzed with coil.We suggest that isolated pulmonary AVM might be one of the rare causes of stroke especially in young adults. 대한뇌졸중학회지제6권제1호2004년5월
