기능부전 혈액투석동정맥루에서 경피경관혈관확장술 후 장기개통율에 미치는 임상인자에 대한 연구

초록

목적 : 기능부전 혈액투석 동정맥루의 치료목적으로 시행한 경피경관혈관확장술(Percutaneous transluminal angioplasty: PTA)의 성공율과 개통율을 알아보고, 이에 영향을 미치는 인자에 대해 알아보고자 본 연구를 시행하였다. 방법 : 기능부전 혈액투석 동정맥루로 PTA를 시행한 73명의 환자(총 92예)를 대상으로 하여 초기 성공율 및합병증을 평가하였다. 대상 환자를 나이와 성별, 투석기간, 당뇨병의 유무, 동정맥루 위치 및 수술 횟수 그리고 혈전의 동반유무에 따라 두 그룹으로 나누어 장기 개통율을 Kaplan Meier method 및 log-rank test를 사용하여 비교하였다. 또한 Cox regression model을 이용하여 장기 개통율에 관여하는 인자를 평가하였다. 결과 : 시술의 초기 성공률은 78.2% (72/92)였으며, 성공한 예의 추적검사에서 6개월, 12개월 개통율은 80.0%와 73.2%였다. 환자의 나이, 성별, 투석기간, 당뇨병의 유무, 동정맥루 위치 및 혈전의 동반유무 등이 장기개통율에 미치는 영향은 통계학적으로 유의하지 않았다. 2회 이상의 동정맥루 수술을 시행한 경우 6개월, 12개월 개통율이 62.3%와 48.5%로 1회의 동정맥루 수술을 시행한 예의 6개월, 12개월 개통율 87.8%와 80.5%보다 낮았다(p=0.026). PTA와 관련된 가장 흔한 합병증은 혈관파열(1.3%)과 혈종(3.9%)이였다.

Abstract

Background : The aim of this study is to evaluate success rate and patency rate after percutaneous transluminal angioplasty (PTA) and to determine the factors affecting patency rate afterPTA in the management of insufficient arteriovenous fistula of hemodialysis patients. Methods : Ninety-two cases of insufficient arteriovenous fistulae in 73 hemodialysis patients underwent angiography of the fistula and were treatedby PTA (native AVF: 67 cases, AV graft: 10 cases, central vein: 15 cases). The initial success rate and complications of PTA were evaluated. All patients were divided into two groups according to the age, sex, the duration of hemodialysis, a history of diabetes mellitus, the numbers of AVF operation, and the presence or absenceof thrombus. The patency rate was compared with the Kaplan-Meier method with log-rank test. To determine the clinical factors that affect vascular access failure rate, Cox regression method was used. Results : The initial success rate of PTA was 78.2% (72/92). The patency rate after successful PTA was 80.0% at 6 months, and 73.2% at 12 months. The patency rate after successful PTA was significantly lower in cases which had two or more AVF operation (62.3% and 48.5% at 6 and 12 months) than just one (87.8% and 80.5% at 6 and 12 months). The most common PTA related complications were vessel spasm (3.9%) and hematoma (3.9%). Conclusions : The numbers of AVF operation was the only risk factor which affected the patency rate.(Korean J Med 70:551-558, 2006)
