동정맥루 혈관성형술 후 발생한 피부괴사를 동반한 심한 도류증후군 1예

본 증례는 당뇨병성 신부전으로 혈액투석을 받고 있는 65세 여자 환자에서 동정맥루 문합부 협착에 대한 혈관성형술직후 발생한 도류증후군이 동정맥루 결찰을 통해 호전된 예이다. 환자는 6개월 전 우측 상완에 동정맥루 수술을 받았고,혈관조영술에서 동정맥루 문합부에 협착증이 발견되어 혈관성형술을 시행했다. 그러나 그 후 발생한 동정맥루 원위부의허혈성 통증과 피부궤양을 주소로 내원하여 도류증후군으로진단받고 동정맥루를 결찰했다. 혈관성형술 후 발생하는 도류증후군은 발생 빈도가 낮으나 조기에 발견하여 치료하지않으면 손의 궤양이나 운동 장애에 이를 수 있다. 또한 괴사까지 진행하여 심한 경우 수부의 절단까지 필요할 수 있어발생 가능성을 반드시 인지하고 있어야 하며, 신속한 진단과치료가 필수적이다.
Abstract
Arterial steal syndrome is a rare but serious complication that disrupts antegrade flow distal to an arteriovenous fistula (AVF) becauseof excess blood flow through the AVF. A 65-year-old woman with diabetes mellitus and undergoing hemodialysis was admittedfor coldness and pain in the right hand ipsilateral to an AVF. AVF stenosis had developed 6 months after an upper-arm AVFoperation. These manifestations developed 2 days after a successful radiological intervention for a stenotic lesion in the AVF,which became worse until the skin on her hand ulcerated. The symptoms became aggravated, particularly during dialysis. Fistulography revealed that the AVF anastomosis site was patent but blood flow toward the forearm had decreased severely. Arterial steal syndrome developing after percutaneous angioplasty for an AVF stenosis was suspected, and the AVF was ligated,which resolved the hand pain and ulceration.
