Acute cerebellar infarction involving the territory of bilateral posterior inferior cerebellar artery(PICA) is rare. The pathogenesis of the simultaneous bilateral cerebellar infarction is still unclear. We describe 2 patients with sudden onset bilateral ataxia and vertigo. Brain MRI revealed bilateral cerebellar infarction in the territory of the whole PICA in one patient and both medial PICA in the other patient. On transfemoral cerebral angiography, the patient with bilateral whole PICA infarction showed absent right PICA and abrupt cut off at the proximal portion of the left PICA. The other patient with bilateral medial PICA infarction showed severe stenosis of the right proximal PICA. This suggests that bilateral PICA infarction resulted from an occlusion of one PICA that supplied bilateral PICA territory. Korean Journal of Stroke 1999;1(1):83~89
