Background : Tumors originating from the medulla oblongata are very rare, and may present with lateral medullary syndrome(LMS). We describe two such patients. Case Report s : A 59-year-old man developed vertigo, vomiting and truncal ataxia. MRI showed a high signal intensity lesion involving dorsolateral area of the right medulla oblongata. Repeat MRI revealed two enhanced mass lesions. An open biopsy showed findings consistent with malignant lymphoma. The other 47-year-old man suddenly developed vertigo, left Horner sign, right falling tendency, and right-sided hypesthesia including the face. Brain MRI demonstrated an enhanced exophytic mass involving the left medulla. Although a biopsy was not performed, enlarged mass on follow up MRI and progressively deteriorated clinical course were highly suggestive of malignant brain tumor. Conclusion : We suggest that follow up MRI or brain biopsy should be performed in selected patients who presented with atypical LMS. Korean Journal of Stroke 2001;3(2): 171~176
