심장재동기화치료는 증상 있는 심한 심장기능상실 환자에서 유용한 한 치료법이지만 심장정맥굴을 통한 조율유도 삽입과 관련되어 합병증이 발생할 수 있다. 저자들은 심장재동기화치료를 위해 심장정맥굴 조율유도를 삽입하던 중 심장정맥굴 박리가 발생하였지만 성공적으로 조율유도 삽입한 1예를 경험하였기에 보고하는 바이다.
Abstract
Cardiac resynchronization therapy is effective in selected patients with symptomatic left ventricular dysfunction. Coronary sinus dissection is a relatively uncommon, but much feared, complication that may occur during placement of the left ventricular pacing lead. A 68-year-old man was transferred to our hospital for treatment of severe heart failure. He developed coronary sinus dissection after balloon dilatation during the procedure for cardiac resynchronization therapy. Nevertheless, we successfully placed the left ventricular pacing lead in the coronary vein. We present the case with a review of the literature. (Korean J Med 2011;80:333-336)
