변비의 진단에 있어 일차적으로 기왕력 청취 및 관련 증상의 파악, 이학적 검사 등이 중요하지만 기질적 또는 기능적 원인에 의한 변비 감별에는 도움을 주는 데 한계점이 있을 수 있다. 하지만 40세 이상 또는 젊은 연령에서 대장암의 가족력이 있는 환자들 중에서 혈변, 지속적인 가는 변, 체중감소, 식욕저하를 동반하는 복통 등 경고 증상이 있을 경우에는 기질적 질환을 배제하기 위한 검사가 필요하다. 변비의 원인이 기능성으로 추정되는 경우, 그 병태생리를 규명하고 치료 방침을 결정하기 위해 자세한 병력 청취를 비롯한 수지직장검사를 통하여, 보다 정밀한 검사가 필요할 경우, 대장항문 기능검사 및 대장통과시간 측정 등이 필요하며 이러한 여러 기능 검사를 토대로 진단 및 치료의 방향을 결정해야 한다.
Abstract
Constipation is a common symptom affecting approximately a quarter of population in Korea. To diagnose its etiology and to exclude the other organic or structural causes, many tests including digital rectal exam, laboratory tests, image studies, and physiologic studies are needed. Moreover, through these tests, the exact pathophysiology of constipation can be defined and used in the treatment of patients. Therefore, collaborative studies are necessary to determine the pathophysiology of this disorder. (Korean J Med 2011;80:505-509)
