초록

위식도 역류 질환(gastroesophageal reflux disease, GERD)은위의 내용물이 식도로 역류하면서 발생하는 질환이다[1]. 서구에서는 20% 정도로 상당히 높은 유병률이 보고되는 질병으로[2], 우리나라에서는 유병률이 11.8-17.3%로 보고되나[3,4],서구화된 생활 습관으로 인해 향후 현재보다 증가할 것으로예측된다. 위식도 역류 질환에 대한 Motreal International Consensus[1]와 기능성 위장관 질환에 대한 로마 기준 III [5]의 도입으로 위식도 역류 질환 중 비미란성 역류 질환의 개념은 변화하게 되었으며, 그 정의도 계속해서 진보하고 있다. 위식도 역류 질환의 높은 유병률 및 역류 질환과 과민성장증후군과의 강한 연관성은 비미란성 역류 질환의 영향에기인한다. 최근까지 위식도 역류 질환이라 하면 미란성 식도염을 떠올리게 되는데, 그 이유는 점막 염증의 치유가 식도염치료의 객관적인 지표로 느껴지기 때문이다. 비미란성 역류질환은 점막 병변이 없음에도 불구하고 위식도 역류 질환과유사한 삶의 질 감소가 관찰되므로, 단순히 가벼운 역류성 질환으로 치부할 수는 없다. 더구나 비미란성 역류 질환은 산억제를 통한 증상 조절이 미란성 식도염보다 어렵다. 본고에서는 비미란성 역류 질환의 개념과 위식도 역류 질환 및 기능성 위장관 질환의 관계에 대하여 살펴보고자 한다.

Abstract

Nonerosive reflux disease (NERD) is a distinct presentation of gastroesophageal reflux disease (GERD). NERD is commonlydefined as the presence of classical GERD symptoms in the absence of esophageal mucosal erosions/breaks on upper endoscopy. Within the spectrum of GERD, the pathophysiological and clinical characteristics distinguishing NERD and erosive esophagitis remaina matter for debate. Recent research on NERD has focused on understanding the pathophysiology and natural history. Giventhe increased incidence of NERD in Korea, we reviewed the concept of NERD and its relationship to GERD and functional gastrointestinaldisorders.
