HFpEF를 포함하는 심부전은 전 세계적으로 중요한 공중보건 문제이며 고령화 시대에 앞으로 더욱 증가할 것으로예상되고 있으나 아직 우리의 이해나 관심은 부족한 편이므로 예방, 진단, 치료에 보다 적극적인 대처가 필요하다. HFpEF의 치료를 위해서는 좌심실 수축 기능 감소 심부전증과 다른 병태생리에 대한 인식과 이완 기능 장애 평가 방법에 대한 포괄적인 이해, 동반 질환에 대한 적극적인 관리가이루어져야 하며 생존율 개선을 위한 새로운 치료가 개발되어야 할 것이다.
Abstract
More than 50% of patients who are diagnosed with heart failure have preserved ejection fraction (HFpEF), and they have anequally poor prognosis when compared to patients with heart failure with reduced ejection fraction (HFrEF). However, a comprehensiveunderstanding and awareness of heart failure with preserved ejection fraction is still limited and there are currently no optimizedtreatments to improve morbidity and mortality in these patients. This review summarizes the differences in the epidemiology,pathophysiology, diagnosis, and prognosis between HFpEF and HFrEF. We also review current management strategies of HFpEFpatients according to evidence-based treatment guidelines.
