임신 여성의 산전 자가간호 행위 영향요인

본 연구는 임신 여성의 산전 자가간호 행위에 영향을 미치는 요인을 확인하기 위하여 시도되었다. 연구 대상은 일 도시 소재 M 여성전문병원의 외래를 방문한 임부 158명이었으며, 설문조사는 2017년 7월 1일에서 8월 1일까지 실시하였다. 자료수집은 산전우울(에딘버러 산후우울증 척도), 임부태아애착 및 산전 자가간호 행위 척도를 사용하였다. 수집된 자료는 SPSS WIN 23.0 프로그램을 사용하여, 기술통계, t-test, one-way ANOVA와 Sheffe’s test, Pearson’s correlation coefficients 및 multiple regression으로 분석하였다. 산전 자가간호 행위는 임신 횟수(r=-.18, p=.023)와 산전우울(r=-.35, p<.001)과 유의한 음의 상관관계를, 임부태아애착과는 유의한 양의 상관관계(r=.18, p=.019)를 보였다. 산전 자가간호 행위 영향요인은 산전우울, 임부태아애착 및 임신 횟수로 나타났으며, 이들 변수에 의한 임신 여성의 산전 자가간호 행위에 대한 설명력은 15.4%이었다. 따라서 임신 여성의 산전 자가간호 행위를 증진시키기 위하여, 임신 여성의 우울을 미리 선별하여 우울 증상을 관리하고, 임부태아애착을 증진시키기 위한 방안들을 마련해야 할 것이다. 또한 임신 횟수가 많아질수록 자가간호에 더 소홀해지므로, 초임부 보다는 경임부의 자가간호 행위에 보다 높은 관심을 가져야 하겠다.
Abstract
The purpose of this study was to identify the influencing factors on prenatal self-care behavior among pregnant women. The subjects of this study were 158 pregnant women visiting outpatient clinic of M obstetric and gynecologic hospital in a city, and this study was conducted from July 1, 2017 to August 1, 2017. Data were collected through EPDS (Edinburgh Postnatal Depression Scale), maternal-fetal attachment, and prenatal self-care behavior. The collected data were analyzed by descriptive statistics, t-test, one-way ANOVA and Sheffe’s test, Pearson’s correlation coefficients, and multiple regression using SPSS WIN 23.0 program. Prenatal self-care behavior was a significant negative correlation with frequency of gravida and antepartum depression and a significant positive correlation with maternal-fetal attachment. The influencing factors on prenatal self-care behaviors were antepartum depression, maternal-fetal attachment, and frequency of pregnancy, and these variables explained 15.4% of all variance of prenatal self-care behavior among pregnant women. Therefore, to promote prenatal self-care behavior among pregnant women, it is necessary to screen and manage early depression of pregnant women, and to prepare measures to promote maternal-fetal attachment. In addition, more attention for prenatal self-care will be needed to multigravida than primigravida.
