임신과 동반된 침윤성 자궁경부암에서 Paclitaxel-Cisplatin병합요법 후 성공적인 임신 1예와 문헌 고찰

임신 중 악성 종양이 동반되는 경우는 매우 드물며, 진단되는 경우 그 치료 방법의 선택에 있어서 임산부와 태아 모두에게 미칠 수 있는 영향을 고려해야 하므로 많은 어려움이 따른다. 본 증례는 임신 중 진단된 침윤성 자궁경부암 환자에서 임신 중 항암화학요법으로 2차례의 paclitaxel-cisplatin을 투여받은 후 임신 36주에 고식적 제왕절개술과 양측 부대동맥 림프절 제거술을 시행받았던 예로, 출산 후 신생아에서 선천성 기형은 없었으며 생후 4.3개월에 소아과 외래에서 진찰시 6.4 kg의 정상 발육과 발달 소견을 보였다. 이와 같은 증례는 아직까지 문헌 보고가 없었기에 문헌 고찰과 함께 보고하는 바이다.
Abstract
Paclitaxel-Cisplatin based chemotherapy in invasive squamous cell carcinoma of cervix during pregnancy has not been reported previously. A woman was diagnosed with stage Ⅱb invasive squamous cell carcinoma of cervix during pregnancy at 27 weeks gestational age. She was treated with two cycles of paclitaxel and cisplatin during pregnancy. At 36 weeks, she underwent a classical cesarean section and bilateral paraaortic lymphadenectomy. One additional cycles of chemotherapy and radiotherapy were given. She developed a recurrence within 6 months of surgery. The infant has normal growth and development at 4.3 months of age. This is the first reported case of paclitaxel-cisplatin use in cervical cancer during pregnancy.
