임신 중 자궁경부암의 처치

초록

=자궁경부암은 임신 중 진단되는 가장 흔한 부인암이다. 자궁경부 세포진 검사는 산전 진찰의 중요한 부분이기 때문에, 임신은 자궁경부암을 조기에 진단할 수 있는 기회를 제공해 준다. 비정상적인 세포진 검사 결과가 나오면 비임신시와 마찬가지로 처치한다. 즉, 비정상 세포진 검사 소견이나 질출혈 등의 증상이 있다면 질 확대경하 조준 생검을 고려하여야 한다. 임신 중 원추절제술의 유일한 적응증은 미세침윤암을 배제하거나 침윤암을 진단함으로써 치료시기와 분만방법을 결정해야 하는 경우이다. 임신 중 자궁경부암은 일반 환자군에 비해 초기에 진단된다. 초기 자궁경부암 환자에서, 암 병변이 진행하는 소견이 없다면 태아의 성숙을 위하여 치료를 연기하는 것이 가능하다. 임신 중 자궁경부암의 치료는 비임신 환자와 동일한 원칙이 적용되지만 병기, 임신 주수 및 임신 유지에 대한 환자의 희망 등을 고려하여 선택되어야 한다. 임신 중 자궁경부암 환자의 적절한 치료를 위해 부인종양 전문의뿐만 아니라 소아과 및 산과 전문의가 참여하는 협진 체계가 필요하다. 임신 중 자궁경부암 환자의 예후는 비임신 환자와 비슷하다.
