난소임신은 자궁외 임신의 비교적 드문 형태로 최근 혈청 β-hCG에 대한 정량 검사와 경질 초음파로 조기진단이 되는 경우도 있지만, 수술 전에는 진단이 어려운 경우가 많고, 복강내 대량출혈과 동반되어 황체 파열로 오진하기 쉽다. 확진은 수술 후 병리조직학적 검사로 한다. 치료는 수술이 원칙이며 최근에는 복강경의 발달로 보존적 수술을 시행하기도 한다. 저자들은 난소임신 1예를 경험하였기에 문헌 고찰과 함께 보고하는 바이다.
Abstract
Ovarian pregnancy is comparatively rare form of ectopic pregnancy. Although earlier diagnosis is now possible due to the availability of quantitative β-hCG measurement and the development of transvaginal ultrasonograghy, it is mostly difficult to diagnosis before surgery, and frequently misdiagnosed as a ruptured corpus luteum accompanied with massive hemoperitoneum. Definite diagnosis is made only by cytopathologic examination of tissue specimen. The treatment of ovarian pregnancy has been operative management including oophorectomy, salpingo-oophorectomy and ovarian wedge resection. But recently conservative management using laparoscopic technique has become the preferred treatment. We have experienced a case of primary ovarian pregnancy and reviewed it briefly.
