자궁경관 임신 및 제왕절개 반흔부 임신에서의 methotrexate 치료 효과 및 예후 예측 인자

The efficacy and prognostic factors of methotrexate treatment in cervical/previous cesarean scar pregnancy

초록

=목적: 자궁경관 임신 또는 제왕절개 반흔부 임신에 있어서 methotrexate를 이용한 화학 요법시 치료효과 및 성공 예측인자를 알아보고자 하였다. 연구 방법: 자궁경관 임신 또는 제왕절개 반흔부 임신으로 진단받은 후, 생체 징후가 안정되고 과도한 질 출혈이 없어 methotrxate 치료를 받은 환자 25명을 대상으로 후향적 조사를 통하여 결과를 분석하였다. 치료는 국소적 요법으로는 경질 초음파 유도하에 태낭내에 methotrexate 50 mg을 직접 투입하였거나, 전신적인 요법으로는 methotrexate 1 mg/kg을 격일로 근주하였다. Methotrexate 투여후 치료 결과는 1주 간격으로 혈중 β-hCG를 측정하여 5 mIU/mL 미만으로 감소시까지 경과 관찰하였다. 치료실패군과 성공군 간의 임상적 특성, 치료시작시의 β-hCG 농도, 초음파 검사소견, methotrexate 투여경로를 비교하였다. 통계분석은 윈도우용 SPSS를 이용하여 Student T-test, Pearson's χ2 test, Fisher's exact test를 시행하였으며 통계적 유의성은 P value 0.05 미만으로 하였다.결과: 전체 25명의 환자 중 16명 (64%)의 여성이 methotrexate 단독치료만으로 성공적으로 치료되었으며, 추가적인 수술을 받은 후 가임력을 유지한 경우는 96%였다. 치료실패군과 성공군간의 비교에 있어서 환자의 나이, 분만력, 자궁 소파술 과거력, 제왕절개술 과거력, 임신주수, 치료시작시 β-hCG 농도, 초음파 소견 (임신낭, 난황낭, 태아 음영, 태아 심박동이 확인된 경우) 등은 통계학적으로 유의한 차이가 없었다. 그러나 진단당시 이미 혈종이 형성된 9예 중 6예에서 methotrexate 치료에 실패함으로써 두 군간의 유의한 차이가 있었다. 또한 methotrexate 투여 방법에 있어서도 국소 요법을 시도한 13예 중 11예에서 치료에 성공한 반면, 전신적 요법을 시도한 12예 중 7예에서 치료에 실패하여 통계학적으로 유의한 차이가 있었다.
