TVT (Tension free vaginal tape)와전자궁 적출술 병합에 대한 임상연구

초록

=목적: 전신마취하에 복압성 요실금 치료에 이용되는 무장력 중부 요도 현수술 (tension free vaginal tape, 이하 TVT)과 전자궁 적출술 병합시 TVT의 효율성, 안정성 및 합병증을 평가하고자 하였다. 연구 방법: 2003년 1월부터 2003년 12월까지 TVT와 전자궁 적출술을 병합한 76예의 환자를 대상으로 후향적으로 분석하였다. 수술전 병력청취, 이학적 검사, 골반 내진, 1-hour pad test 및 요역동학적 검사를 시행하였으며, 모든 수술은 전신마취하에 시행되었다. 65예에서 LAVH (laparoscopic assisted vaginal hysterectomy)와 병합하였으며, 6예에서 VTH (vaginal total hysterectomy)와 병합하였고, 5예에서 TAH (Total abdominal hysterectomy)와 병합하였다. 결과: 평균 추적관찰 기간은 13개월 (range 6-18개월) 이었으며, 평균 입원 기간은 7.6일 (range 6-15일) 이었다. 객관적인 성공률은 97.4% (완치 92.1%, 호전 5.3%) 였으며, 주관적인 성공률은 97.4% (완치 88.1%, 호전 9.3%) 였다. 수술전후의 1-hour pad test는 각각 36.4 gm, 4.1 gm으로 (p<0.001)이었다. 방광 천공, 수술 후 배뇨장애 및 수술 후 과민성 방광 증후군 등의 합병증을 각각 6.6%, 7.9% 및 7.9%에서 보였다.
