복압성 요실금 수술시 tension-free vaginal tape의 치료 성적; 요도 괄약근 부전증 환자와 비요도 괄약근 부전증 환자의 비교

초록

=목적: 요도 괄약근 부전증인 요실금 환자와 비요도 괄약근 부전증인 요실금 환자에서 TVT 시행 후 치료 성적을 비교하고 이를 통해 TVT의 요실금 환자 치료에 있어서의 치료 효과에 대해 평가 하고자 하였다. 연구 방법: 2003년 6월부터 2005년 6월까지 연세대학교 세브란스병원 산부인과에 내원하여 복압성 요실금으로 진단 후 TVT를 시행받은 111명의 환자를 대상으로 후향적으로 연구하였다. 요실금은 요역동학 검사에 의해 진단되어 졌으며 복압상승시 배뇨근 수축없이 불수의적으로 요실금이 있는 경우 복압성 요실금으로 정의하였다. 요역동학 검사 (Dantec- 5000 system, Copenhagen, Denmark)는 uroflowmetry, multichannel cystometry, urethral pressure profilometry로 이루어졌다. ISD는 MUCP<20 cmH2O이거나 VLPP<60 cmH2O인 경우로 정의하였고 NISD는 MUCP>20 cmH2O이고 VLPP>60 cmH2O인 경우로 정의하였다. 환자들은 수술 후 1, 3, 6, 12개월 후 추적관찰하였다.결과: 평균연령은 요도괄약근 부전증이 있는 환자군에서 60.74±10.00세, 비요도괄약근 부전증 환자군에서 57.20±10.94세로 두 그룹간에 차이가 없었다 (P>0.05). 분만력, 신체질량지수 (BMI), 폐경된 환자의 비율, 호르몬 치료 시행받은 기왕력이 있거나 현재 호르몬 치료중인 환자의 비율 모두 두 그룹간에 통계학적으로 유의한 차이가 없었다 (P >0.05). 방광 충만 검사상 뇨의를 처음 느꼈을 때의 방광 용적, 정상 뇨의를 느꼈을 때의 방광용적, 강한 뇨의를 느꼈을 때의 방광용적, 최대 방광용적 모두 두 그룹간에 통계학적으로 유의한 차이가 없었다 (P>0.05). MUCP는 요도괄약근 부전증이 있는 환자군에서 46.81±24.29, 비요도괄약근 부전증 환자군에서 75.70±32.61로 두 그룹간에 차이가 있는 것으로 나타났다 (P<.0001). VLPP는 요도괄약근 부전증이 있는 환자군에서 53.48±10.12, 비요도괄약근 부전증 환자군에서 107.23±42.95로 두 그룹간에 차이가 있는 것으로 나타났다 (P<.0001). 요류검사상 최대요속, 평균요속, 총 요량에서도 두 그룹간에 통계학적으로 유의한 차이가 없었다 (P>0.05). 수술 후 1, 3, 6, 12개월 후 두 그룹간에 완치율을 비교한 결과 수술 1개월 후 요도괄약근 부전증이 있는 환자군에서 87%, 비요도괄약근 부전증 환자군에서 100%로 두 그룹간에 통계학적으로 유의한 차이가 있는 것으로 나타났으며 (P=0.0053) 수술 3, 6, 12개월 후 두 그룹간에 통계학적으로 유의한 차이가 없었다 (P>0.05). 수술 후 합병증 발생률을 비교한 결과 배뇨곤란, 새로 발생한 절박뇨 (de novo urgency), 요로 감염 발생은 추적 관찰기간에 관계없이 두 그룹에서 통계학적으로 유의한 차이가 없는 것으로 나타났다 (P>0.05).
