여성 복압성 요실금의 수술시 transobturator tape술식에서 ‘outside-in’ (TOT) 수술법과‘inside-out’ (TVT-O) 수술법의 비교 연구

초록

=목적: 여성 복압성 요실금의 수술 방법인 TOT와 TVT-O의 효과를 후향적으로 비교하고자 한다.방법: 2003년 1월 1일부터 2006년 6월 30일까지 본원에 내원한 복압성 요실금 환자 중 184명 (TOT)과 136명 (TVT-O)의 환자별 임상적 특성, 수술 전 실시했던 요역동학 검사와 Q-tip 검사, 수술의 성취도, 평균 수술시간, 수술 후 평균 재원기간, 수술 후 합병증 등을 서로 비교 연구하였다.결과: Q-tip test에서 두 군 모두 요도의 과운동성을 보였으며, 요역동학 검사에서 평균최대요속과 잔뇨량은 두 군간에 유의한 차이가 없었다. 내인성 요도괄약근 부전증은 TVT-O군에서 유의하게 높았다. 수술의 성취도 는 각각 3명 (TOT), 4명 (TVT-O)에서 치료 실패를 보인 것 이외에 모두 수술이 성공적이었다. 수술시간, 입원기간, 헤모글로빈 변화량은 두 군에서 유의한 차이가 없었다. 합병증으로 TOT군에서는 수술 부위 미란이, TVT-O군에서는 대퇴부 혹은 질 통증이 가장 많았다. 그 외에 요정체, 절박뇨, 요로감염 등이 관찰되었다. 수술 후 합병증 발생은 TOT군과 TVT-O군에서 통계학적 차이는 없었다. 동반수술 없이 TOT만 시행한 경우와 질벽 교정술을 함께 시행한 경우 합병증 발생의 차이는 유의하지 않았다. TVT-O군에서도 유의한 차이를 보이지 않아 질벽 교정술을 동반했다고 해서 합병증의 발생률이 증가하지는 않았다. TOT군에서 내인성 요도괄약근 부전증이 없었던 경우와 있었던 경우의 합병증 발생률에 유의한 차이는 없었다. TVT-O군에서도 내인성 요도괄약근 부전증이 없었던 경우와 있었던 경우의 합병증 발생률에 유의한 차이는 없었다. 따라서 합병증 발생률과 내인성 요도괄약근 부전증 여부와는 큰 관련성이 없다고 볼 수 있었다.
