Abstract
Precision is defined as the closeness of agreement between the results of multiple estimations of the same or similar objects, obtained through replicate measurements under specified conditions. Precision can be categorized into repeatability, within-laboratory precision, and reproducibility. Each type of precision provides insight into the reliability and consistency of a measurement procedure under different circumstances. Precision evaluation is a performance assessment performed during the initial setup of measurement procedures in clinical laboratories, involving challeng- es related to the relative complexity of the study design and calculations. The Clinical and Laboratory Standards Institute (CLSI) EP05-A3 and EP15- A3 provide standardized frameworks for evaluating precision performance of quantitative measurement procedures. CLSI EP05-A3 focuses on es- tablishing or validating precision performance, offering recommendations targeted at manufacturers and developers. In contrast, CLSI EP15-A3 emphasizes verification of the manufacturer’s precision claims, providing practical guidance for end-user laboratories to ensure measurement re- liability. CLSI EP05-A3 employs a 20×2×2 design over 20 or more days using two-way nested ANOVA, while CLSI EP15-A3 utilizes a 5×5 design over five days with one-way ANOVA. Both guidelines provide essential estimates of repeatability and within-laboratory precision. This study is ex- pected to contribute to improving the quality of diagnostic tests in Korea by enhancing the understanding of how precision performance estimates are established and interpreted for quantitative measurement procedures across clinical laboratories and manufacturers. As a result, it will be- come easier and more efficient to apply the methodology.
