일부 지역주민의 암 조기검진율과 관련요인

연구배경: 본 연구는 일부지역 주민을 대상으로 우리나라에서 많이 발생하는 암의 조기검진율에 영향을 미치는 요인을 파악하여 지역사회에 적합한 암 조기검진 프로그램 개발을 위한 기초자료를 제시하고자 하였다. 방법: 일개 광역시 1개 구와 일개 도 1개 군의 주민 중에서 행정단위인 반을 대상으로 무작위집락 추출하였다. 추출된 반에 거주하는 30세 이상의 주민 2,157명을 대상으로 구조화된 설문을 사용하여 훈련된 조사원에 의해 직접 면접 설문조사를 실시하였다. 암 조기검진율을 파악하고 이와 관련된 요인을 앤더슨 모형(Andersen model)을 적용하여 분석하였다. 결과: 최근 3년간 한번이라도 조기검진을 받아 본 사람들의 검진율은 위암이 8.9%, 간암이 10.5%, 대장암이 4.5%, 자궁경부암이 46.3%, 유방암이 16.0%이었다. 앤더슨 모형을 적용하여 암 조기검진과 관련된 요인을 분석한 결과 소인성 요인(predisposing factors)으로는 위암 조기검진의 경우 성별, 연령, 교육수준이, 간암 조기검진의 경우 성별과 연령이, 대장암 조기검진의 경우 성별, 연령, 교육수준, 그리고 결혼상태가, 자궁경부암의 조기검진의 경우 연령, 교육수준, 그리고 결혼상태가, 유방암 조기검진의 경우 연령이 유의하게 나타났다. 가능성 요인(enabling factors)으로는 위암 조기검진의 경우 경제수준이, 자궁경부암 조기검진의 경우 지역과 경제수준이, 유방암 조기검진의 경우 지역이 유의하게 나타났다. 필요요인(needs)으로는 위암 조기검진의 경우 암과 조기검진에 대한 지식 그리고 음주가, 간암 조기검진의 경우 암과 조기검진에 대한 지식, 흡연, 그리고 음주가, 대장암 조기검진의 경우는 암과 조기검진에 대한 지식, 흡연, 그리고 음주가, 자궁경부암 조기검진의 경우 암 조기검진에 대한 태도가, 유방암 조기검진의 경우 암 조기검진에 대한 태도와 암의 가족력이 유의하게 나타났다. 결론: 본 연구에서 유의하게 나타난 변수 가운데 성별, 연령, 교육수준, 결혼상태 등은 개인, 사회의 노력에 의해 수정하기 어려운 변수(incorrectable)인데 비해, 지식 및 태도, 음주 및 흡연과 같은 건강행태 등은 수정 가능한 변수(correctable)이다. 따라서 지역사회 암 조기검진율을 높이기 위해서는 암 조기검진에 대한 지식과 태도의 변화 및 건강행위실천을 촉진시킬 수 있는 방안을 개발해야 할 것이다.
