폐쇄성 수면무호흡증의 임상양상, 진단 및 치료

초록

폐쇄성 수면무호흡증은 심혈관계 합병증, 인지기능 저하, 사회․행동적 장애를 야기하는 심각하고 만성적인 질환이다. 수면무호흡증에서의 수면 중 반복적인 상기도의 허탈로 인해 산소포화도가 저하되고 수면이 분절되어 피로감, 주간 졸음, 두통, 인지기능 저하 및 운동능력 저하가 야기되고 사고의 위험성이 증가한다. 폐쇄성 수면무호흡증은 젊은 성인의 경우 남자에서 4%, 여자에서 2% 정도의 유병률을 보이나 나이가 들어감에 따라 유병률이 증가하고 특히 여성에서 이 증가 폭이 커서 노인에서는 남녀 간 유병률 차이가 감소한다. 연령의 증가, 비만, 특히 중추성 비만, 굵은 목, 고혈압, 폐쇄성 수면무호흡증의 가족력, 두개안면 이상(craniofacial abnormalities), 취침 전 음주 등은 남․여, 성인․노인 모두에서 공통된 폐쇄성 수면무호흡증의 위험요인이다. 병력 청취, 두경부와 비강 및 구강 구조에 대한 이학적, 방사선학적 검사가 도움이 되며 정확한 진단과 치료를 위해서는 수면다원검사가 필수적이다. 치료법에는 지속적 상기도 양압요법, 구강 내 장치, 행동요법 등의 비수술 치료법과 수술 치료법이 있다. 치료법의 선택은 수면 무호흡의 정도, 비강, 구강 및 인후부의 구조, 주간 증상, 심폐기능 등을 고려하여 결정한다.
