하지불안증후군 (Restless legs syndrome, RLS)은 비교적 흔한 질환으로 다리에 움직이고 싶은 충동과 함께 대부분 불편한 증상이 동반되며, 이러한 증상은 쉬거나 가만히 있을 때 악화되고, 움직이면 호전되며, 야간에 악화되는 것을 특징으로 하는 감각운동 신경장애다. 또한, RLS는 다리의 불편한 증상과 더불어 약 70∼80%에서 수면장애를 동반한다. 우리나라에서는 약 7.5%의 유병율을 가져 약 300만 명 이상이 이 질환을 가지고 있는 것으로 추정된다. RLS는 다른 질환 없이 생긴 원발성 RLS와 철 결핍성 빈혈, 만성신부전, 임신 등 다른 질환에 동반되어 생긴 이차성 RLS로 구분할 수 있다. RLS 는 임상적 증상을 바탕으로 비교적 간단히 진료실에서 진단할 수 있지만 유사한 다른 질환과 감별진단이 필요하다. RLS는 정확히 진단되어 적절히 치료하면 빠르게 호전되는 질환이다. 치료방법은 비약물요법과 약물요법으로 나뉘어 지며 환자의 증상 심각도 와 이로 인한 사회활동에 미치는 영향 등을 고려하여 치료를 시작하여야겠다. 원인이 되는 질환이 있으면 이를 먼저 치료해주어야 하겠으며 약물치료로는 도파민 촉진제 약물이 일차 약으로 선택된다. 일차진료에 있어 수면장애와 다리에 불편을 호소하는 환자들을 진료할 때 꼭 RLS를 염두 해 두고 진료하는 것이 중요하겠다.
