한국 노인의 일상생활 수행능력 및 도구적 일상생활 수행능력과 관련된 요인: 국민건강영양조사 제3기(2005년) 자료를 중심으로

연구배경: 우리나라는 노인 인구가 급속히 증가하고 있어 향후 고령사회에 대한 대책이 필요하다. 일상생활수행능력 (ADL)과 도구적 일상생활수행능력(IADL)은 노인 기능 상태 와 예후, 사망을 예측할 수 있는 지표로서 유용하다. 본 연구는 국민건강영양조사 제3기(2005년)를 이용하여 노인의 ADL과 IADL 상태를 평가하고 이와 관련성이 있는 요인들을 파악하 고자 하였다. 방법: 2005년 국민건강영양조사에 참여한 3,656명 노인을 대 상으로 노인의 일반적 특성, 주관적 증상, 만성 질환을 조사하 고 ADL과 IADL 기능 상태와 그 관련성을 확인하였다. 결과: 의존적 ADL 노인은 17.2%였고 의존적 IADL 노인은 55.7%였다. 본 연구결과 ADL에 영향을 주는 요인으로 연령이 높을수록, 교육수준이 낮을수록, 의료 급여자나 미가입자, 뇌 졸중, 요실금, 당뇨, 관절염, 폐암이 있을 때 더 의존적이었으 며. IADL에 대해서는 연령이 높을수록, 교육수준이 낮을수록, 배우자가 없는 경우, 남성, 뇌졸중, 관절염, 녹내장이나 백내장 이 있는 경우 더 의존적이었다. 결론: 노인의 신체기능에 여러 요인들이 복합적으로 영향을 미치는 것으로 나타나 노인에 대한 보건복지사업은 이를 고 려한 다각적인 접근방법을 모색해야 할 것이다.
Abstract
Background: It is important to evaluate the functional ability of the elderly for optimal care. This study was conducted to identify factors associated with dependence for activities of daily living (ADL) and instrumental activities of daily living (IADL) in elderly adults in Korea. Methods: A cross-sectional study of data from the third Korea National Health and Nutrition Examination Survey (KNHANES Ⅲ) 2005 was undertaken. The sample consisted of 3,656 participants, 65 years of age or older. Multifactorial regression analysis was used to identify associations between ADL and IADL dependence and sociodemographics, lifestyle, and health history. Results: Among the 629 (17.2%) ADL-dependent and signifi cantly and independently associated factors were age, lower education level, health assistance or no-insurance, stroke, urinary incontinence, diabetes, arthritis, and lung cancer. Among the 1,674 (55.7%) IADL-dependent and significantly and independently associated factors were age, lower education level, single or widowed, male, stroke, arthritis, glaucoma and/or cataract. Conclusion: Functional dependence in older adults was directly related to aging and had multiple determinants. Awareness of these determinants should help design health programs that can identify individuals who are at high risk of losing their independence, and implement interventions for slowing or reversing the process.
