한국 신생아집중치료실 입원 신생아의 임신나이별 생존율의 변화 (2002-2009년)

초록

목적: 한국에서 신생아관리의 발전으로 인해 신생아집중치료실(NICU) 치료를 받은 미숙아나 고위험 신생아의 예후 및 생존율은최근 크게 향상되고 있다. 예전의 연구들에 의하면 한국에서 출생체중별로 극소저체중출생아 및 초극소저체중출생아에서의 생존율도매우 증가된 수준을 보이고 있고, 일본 미국의 수준에 근접하는 것을 발표하였다. 이에 한국에서 임신나이별 생존율 및 치료성적을 파악하고 2002년부터 7년간 변화 양상을 살펴보았다. 방법: 2002, 2007, 2009년도에 보고된 전국 NICU 생존율 자료에서, NICU 입원환자의 임신나이별 생존율을 발췌하였다. 일본Perinatal Care Center Research Network 의 2009년 일본 전국 생존율 보고와 미국 NICHD Neonatal Research Network에서 발표한2003-2007년 보고를 비교용으로 사용하였다. 결과: 전국 NICU 입원 신생아의 2002, 2007, 2009년 임신나이별생존율은 22-23주가 각각 3.4, 19.0, 28.6%, 24주가 21.3, 44.0, 59.3%,26주가 59.4, 77.8, 78.0%, 28주가 79.0, 88.0, 91.2%, 30주가 93.8,95.3, 93.6%, 32주가 95.3, 97.1, 98.2%, 34주가 98.8, 99.0, 99.0%, 36주가 99.2, 99.1, 99.2%, 38주가 99.7, 99.5, 99.2%, 40주가 99.7, 99.4,99.2%, 42주 이상이 100.0, 96.3, 94.7%이었다. 28주 이하의 초미숙아에서 한국(2009), 일본(2009), 미국(2003-2007)의 생존율은 각각77.5, 89.0, 71.6%로 일본이 현저하게 우수하였고 한국은 일본과 미국의 사이에 위치하고 있었다. 결론: 한국에서 NICU에 입원한 신생아의 생존율은 2002년부터2009년까지 괄목할 만한 생존율의 향상을 보여왔다. 본 연구는 한국에서 전국규모의 NICU 입원 신생아의 임신나이별 생존율을 검토한 첫 보고이며, 향후 보다 나은 NICU 치료성적의 개선을 위해 본 자료가 활용되기를 기대한다.

Abstract

Purpose: There has recently been improvement in treatment skills and outcomes of patients in neonatal intensive care units (NICUs)of Korea. According to several studies, such results of very to extremely low birth weight infants were markedly improved in comparison to that of Japan or USA. In speculation, the authors reviewed the survival outcomes by gestational age from 2002 to 2009. Methods: We extracted the survival outcomes of NICU patients by gestational age from the data of previous studies. Data from Japanese Perinatal Care Center Research Network and NICHD Neonatal Research Network of US were used for comparison. Results: The survival rates by gestational age are as follows (2002, 2007, and 2009 respectively): 22-23 weeks (3.4, 19.0, 28.6%),24 weeks (21.3, 44.0, 59.3%), 26 weeks (59.4,77.8, 78.0%), 28 weeks (79.0, 88.0, 91.2%), 30 weeks (93.8, 95.3, 93.6%), 32weeks (95.3, 97.1, 98.2%), 34 weeks (98.8, 99.0, 99.0%), 36 weeks (99.2, 99.1, 99.2%), 38 weeks (99.7, 99.5, 99.2%), 40 weeks (99.7, 99.4, 99.2%), longer than 42 weeks (100.0, 96.3, 94.7%). For infants with a gestational age lesser than 28 weeks, the survival rates of Korea (2009), Japan (2009), and US (2003-2007) were 77.5, 89.0 and 71.6% respectively. The statistics of Korea ranked between those of Japan, USA. Conclusion: The survival rate of neonates admitted to NICU in Korea markedly improved from 2002 to 2009. The present study is the first report of survival rate by gestational age in Korea and will become a fundamental basis for further improvement in neonatal care.
