서울시 지역사회정신보건사업 운영체계 개선방안 연구 - 대상자 의뢰네트워크 분석을 중심으로 -

본 연구의 목적은 공공 지역사회정신보건사업의 민간위탁에 의해 나타나는 결과를 이해관계자의 권력관계에 초점을 두어 구체적으로 분석하는 것이다. 이러한 목적달성을 위하여 서울시 3개 자치구의 지역사회정신보건사업관련 조직 간의 정신장애인 의뢰경로에 대한 네트워크분석을 수행하였으며, 그 분석내용을 토대로 관련전문가 6명과의 면접조사를 실시하여 보다 심층적인 분석을 수행하였다. 연구결과 네트워크 분석을 통하여 첫째, 정신보건센터와 정신의료기관간 퇴원자 관리 네트워크의 부재, 둘째, 정신 보건센터 네트워크활동의 자치구내 행정조직 및 정신의료기관에의 편중성, 셋째, 정신보건센터의 서비스 조정 역할의 미약 등과 같은 지역사회정신보건사업의 문제점이 도출되었다. 그리고 심층면접에서 이러한 문제점의 근본적인 요인이 정신의료기관의 자본축적기제에 의한 것임을 밝혔다. 연구분석의 결과를 토대로 지역사회정신보건정책의 개선방안으로 첫째, 정신보건센터 위탁정책의 폐지, 둘째, 정신장애인의 지역 사회 지지체계의 확충, 셋째, 사업재정으로 바우처 방식의 도입을 제안하였다.
Abstract
This study examines the outcome that results from contracting-out community mental health services from the viewpoint of stake-holders. For this purpose, network analysis was carried out with the community mental health organizations of three boroughs in Seoul. On the basis of the above analysis, qualitative study was conducted with six experts in the relevant area. The results are as follows. First, there is lack of network between mental health center and mental care hospital to manage mentally disabled persons. Secondly, there has been excessive concentration of mental health center network activities on administrative organization and mental care hospital within an autonomous district. Thirdly, mental health center did not coordinate services among community mental health organizations. All these points reveal some of the problems of regional community mental health business. Further, a qualitative study revealed that a fundamental cause of the above problems is commercial profits by the mental care hospitals. On the basis of the above studies and analysis, the following solutions are proposed. First, termination of a policy of delegating mental health center. Secondly, expansion of system to support disabled persons by regional community. Thirdly, introduction of voucher system as a part of funding the relevant business.
