산후 우울증의 한방 치료에 대한 무작위대조군연구 중심의 연구 동향

목적: 산후 우울증의 한방 치료에 대한 무작위대조군연구 중심의 연구 동향을 알아보고자 하였다. 방법: 산후 우울증에 대한 한의학적 치료 효과에 대해 고찰하기 위해 국내와 외국의 검색 엔진을 통해 한국어, 영어, 중국어로 쓰인 산후 우울증 산모들을 대상으로 한 무작위대조군연구 논문을 검색하여 최종적으로 총 11편이 선정되었다. 결과: 1. 논문 중 양약의 효능과 비교한 연구는 총 6편으로, fluoxetine hydrochloride과 비교한 논문이 4편, paroxetine과 mirazapine과 비교한 연구가 각각 1편씩이었으며, 한의학적 치료를 시행하였을 때 양약 단독으로 사용되는 경우보다 부작용이 적고, 산후우울증 치료에 효과적인 것으로 나타났다. 2. 논문 중 한의학적 처치간 혹은 가침치료와 효능을 비교한 연구가 총 5편으로, 귀비탕과 함께 평형침법의 효능을 평가한 연구가 1편, 일반 침치료와 疏肝調神침법을 비교한 연구가 1편, 침치료와 함께 간접구의 효능을 평가한 연구가 1편, 침치료와 음악 전침치료를 비교한 연구가 1편, 침치료와 비침습적가침치료를 비교한 연구가 1편이었다. 3. 산후 우울증의 침구치료는 산후 우울증과 깊은 연관을 갖는 장부를 心, 肝으로 보고 심경의 신문과 간경의 태충 등의 혈위를 활용한 연구가 많았고, 인당, 백회 등 다양한 두부 혈위를 사용한 연구가 다수 있었다. 그 외에도 五臟俞募穴침법을 활용한 연구, 13귀혈을 활용한 연구가 있었다. 4. 산후 우울증은 한의학적으로 鬱症에 해당되기 때문에 疏肝解鬱, 養心安神, 調暢氣機하는 逍遙散과 心脾同治, 氣血同補하는 歸脾湯 등의 한약 치료를 활용한 연구들이 있었다. 5. 산후 우울증을 치료하는 데 침, 한약 외에도 간접구나 음악전침치료 등 다양한 한의학적 치료 방법을 제시하는 연구들이 있었다. 결론: 산후 우울증에 있어 한의학적 치료는 단독으로 시행하였을 때 효과적인 치료가 이루어질 뿐만 아니라 항우울제를 함께 복용하는 경우에도 투여량을 줄이고 치료 기간을 단축하는 데 기여할 것으로 기대되며 앞으로 더욱 체계적으로 계획된 연구를 통해 산후 우울증에 대한 한의학적 치료 방법에 대한 근거를 수립하는 것이 필요할 것으로 사료된다.
Abstract
Objectives: The purpose of this study was to analyze the research trends of randomized controlled trials on Traditional Korean Medicine treatment for postpartum depression in women. Methods: In order to investigate the effect of Traditional Korean Medicine on postpartum depression, randomized controlled trials on postpartum depression were searched using domestic and foreign search engines and a total of 11 studies were selected. Results: 1. There were 6 studies comparing Traditional Korean Medicine treatment methods with western medicine: luoxetine hydrochloride(4), paroxetine(1), and mirazapine(1). 2. There were 5 studies comparing methods of Traditional Korean Medicine or sham acupuncture: 1 study evaluated the efficacy of Balance Acupuncture and Guipi Decoction, 1 study compared the effects of conventional acupuncture and Dispersing Liver Regulating Spirit Acupuncture, 1 study evaluated the efficacy of acupuncture and Heat-sensitive Moxibustion, 1 study compared the effects of conventional acupuncture and Musical Electroacupuncture, and 1 study compared the effects of conventional acupuncture and non-invasive sham acupuncture. 3. Postpartum depression is closely related to Heart and Liver, and there were many studies using acupoints Taichong(LR3) and Shenmen(HE7). Cranial acupoints Baihui(DU20) and Yintang(EX-HN3) were also frequently used. 4. Xiaoyaosan and Guipi Decoction were used in herbal medicine treatment for postpartum depression, 5. Novel treatment methods such as Heat-sensitive Moxibustion and Musical Electroacupuncture were used to treat postpartum depression. Conclusion: Traditional Korean Medicine treatment is a good treatment option for postpartum depression. Further systematic studies are needed to establish the basis for Traditional Korean Medicine treatment for postpartum depression.
