Schwannomas are benign tumors originating from Schwann cells and can present at any location in the body. However, portal hepatic schwannomas are exceedingly rare in daily practice and are often misdiagnosed as other benign or malignant neoplasms. In addition, atypical clinical manifestations and imaging findings, in combination with the complex anatomical nature of this region, are difficult to diagnose preoperatively. In this article, we present the case of a 67-year-old woman with a porta hepatic schwannoma who underwent surgical excision and was histopathologically confirmed. Furthermore, we analyzed the imaging features of porta hepatic schwannomas and reviewed the relevant medical literature.
