고령 환자의 불안정성 대퇴 전자간 골절에 대한 시멘트성 고관절 반치환술 시 근위 대퇴골의 재건

초록

목적: 고령의 불안정성 대퇴 전자간 골절 환자에 대하여 시멘트성 인공 고관절 반치환술을 시행하기 위해 대퇴 근위부 내측 골 결손을 재건한 예들의 단기 추시 결과를 알아보고자 하였다. 대상 및 방법: 고령의 불안정성 대퇴 전자간 골절 환자 중 일정한 적응에 해당되는 일부 환자를 대상으로 대퇴 근위부 골결손을 일시적인 2-3개의 축상 K-강선과 환형 강선으로 고정한 내측 경부 피질골판을 이용하여 재건하고 시멘트를 사용한 양극성 인공 고관절 반치환술을 시행하였다. 총 12예 중 1년 이상 추시 되었던 10예를 대상으로 임상적, 방사선학적 평가를 시행하였다. 결과: 전 체중 보행을 시작한 시기는 평균 술 후 4주로, 한 예를 제외하고 모두 술 후 3개월에 수상 전 상태로 회복되었다. 최종 추시 시 동통에 대한 주관적 점수는 4.2이었다. 정복된 소전자 골편과 재건된 근위 대퇴골은 모두 유합되었다.무증상의 대전자 불유합 3예 이외에 수술 또는 술기과 관련된 합병증은 없었다. 결론: 골다공증과 분쇄가 심한 전자간 골절 환자에서, 근위 대퇴거의 결손을 일시적 K -강선과 환형 강선을 사용하여 재건하고, 시멘트를 사용한 고관절 반치환술을 시행한 예들의 단기 추시는 임상적이나 방사선학적으로 좋은 결과를 보였다.

Abstract

Purpose: This study was performed to evaluate the short term result of the proximal femoral reconstruction for primary cemented bipolar hemiarthroplasty to unstable intertrochanteric fracture in the elderly. Materials and Methods: We reconstructed proximal femoral bone defect with medial neck fragment fixed with circlage wires and two or three temporary axial K-wires for primary cemented bipolar hemiarthroplasty to selected elderly patients with unstable intertrochanteric fracture. Among twelve cases, ten were available after at least a year of follow up. We performed clinical and radiological analyses. Results: It took average four Week for full weight bearing for all patients. All but one patient returned to their preinjury level of activity in three months postoperatively. The final VAS for pain is average 4.2. Lesser trochanteric and reconstructed proximal medial femoral fragments were all united. No clinical complication was noted related to the technique except three asymptomatic greater trochanteric non-unions. Conclusion: The early clinical and radiological results of reconstruction of proximal femoral bone defect with circlage wires and temporary axial K-wires for cemented bipolar hemiarthroplasty to osteoporotic unstable intertrochanteric fracture were satisfactory.
