목적: 대퇴골 두 골단 분리증 환자에서 수술적 치료에 따른 임상적 결과와 합병증에 대하여 알아보고자 하였다. 대상 및 방법: 대퇴골 두 골단 분리증으로 수술적 치료를 시행 받은 총 18명, 22예를 대상으로 하였으며, 남자가 13명,여자가 5명이었고, 평균 연령은 14.2세였다. 수술적 방법에 있어서 총 22예 중 9예에서 정위치 나사못 고정술을 시행하였고, 6예에서 도수 정복 후 나사못 고정술을, 4예에서는 관혈적 정복 후 나사못 고정술을, 그리고 나머지 3예에서는 절골술 후 금속판 고정술을 시행하였다. 고정시 2개의 나사못을 사용한 경우가 11예로 가장 많았고, 3개를 사용한 경우가 4예, 1개를 사용한 경우 4예였다. 결과: 술 후 고관절에 대한 기능 평가는 매우 우수 12예, 우수 6예, 양호 2예, 불량 2예였다. 합병증은 총 5예에서 발생하였으며, 대퇴골 두 무혈성 괴사증 3예, 연골 용해증 1예, 그리고 외반슬 변형 1예였다. 대퇴골 두 무혈성 괴사증은 관혈적 정복 후 2개의 나사못으로 고정한 2예와 만성 고도 분리에서 1개의 나사못으로 정위치 고정술을 시행한 1예에서 발생하였으며, 연골 용해증은 3개의 나사못 중 1개가 대퇴골 두를 관통한 경우에서 발생하였다. 결론: 대퇴골 두 골단 분리증을 치료하는데 있어서 합병증을 최소화하기 위해서는 과도한 정복은 피하고, 한 개의 나사못을 이용한 정위치 고정술이 중요할 것으로 생각된다.
Abstract
Purpose: To evaluate the clinical results and complications of surgical treatment in slipped capital femoral epiphysis. Materials and Methods: We report on 18 patients (22 hips) with slipped capital femoral epiphysis who were treated by surgical methods. There were 13 males and 5 females. The mean age at the time of surgery was 14.2 years. Nine cases were treated by in situ screw fixation; 6 by closed reduction and screw fixation, 4 by open reduction and screw fixation, and 3 by osteotomy of proximal femur and plate fixation. Two-screw fixation was performed in 11 cases, three-screw fixation in 4, and single screw fixation in 4. Results: In functional results, there were 12 excellent cases, 6 good, 2 fair, and 2 poor. As for complications, avascular necrosis developed in 3 cases (by forceful open reduction and two-screw fixation in 2 cases, and by in situ fixation at chronic, severe degree in 1 case), chondrolysis in 1 case (one of three screws protruded out of the femoral head), and genu valgum deformity in 1 case. Conclusion: To minimize complications in the treatment of slipped capital femoral epiphysis, it is important to avoid forceful reduction, and to use in situ single screw fixation.
