Gemella morbillorum은 인간의 구강과 위장관에 정상적으로 상재하는 균이며, 이 균으로 인한 감염은 드물다. 이제까지 G. morbillorum으로 인한 증례는 균혈증, 심내막염, 관절염, 한 장기에 국한되어 나타나는 농양 등이 보고되었으며, 우리나라에서 보고된 증례는 1996년에 보고된 뇌막염 1예 뿐이다. 이 균주에 대한 치료는 대부분의 경우에 1차 치료로 항균요법을 사용하고 있으며. 대부분의 항생제에 감수성이 있다. 또한 심장 판막질환 등의 선행 심질환이 있던 환자에서 G. morbillorum에 의한 균혈증과 심내막염의 경우 판막치완술 등의 수술적인 방법이 사용되기도 한다. G. morbillorum은 정상 면역기능을 갖는 경우에는 정상적인 상재균으로 존재하나, 면역력이 저하되거나 감염을 유발할 수 있는 선행 요인이 동반되는 경우에는 병원균으로 발현하여 전신감염 및 국소감염을 일으킨다. 저자들은 만성 알코올 병력과 충치 및 치주염이 동반된 환자에서 G. morbillorum에 의한 균혈증과 폐렴 1예를 경험하였으며, 항균요법으로 성공적으로 치료하였기에 문헌고찰과 함께 보고하는 바이다.
Abstract
Gemella morbillorum is catalase-negative, facultative, anaerobic, gram-positive cocci which is generally thought to be normal inhabitants of mucous membrane in oral cavity, upper respiratory tract, and genitourinary tract of human. It rarely causes infective endocaditis, bacteremia, pneumonia, and localized infection such as lung empyema, brain abscess, and liver abscess. We report a case of bacteremia caused by G. morbillorum in a 44-year-old man who had been an alcoholic and suffered from dental caries. He was successfully treated with antimicrobial therapy.
