한국 소아청소년에서 고도 비만의 진단

연구배경: 우리나라 소아청소년에서 비만 유병률은 2000 년대 초반 이후 큰 변화 없으나, 고도 비만 유병률은 증가하고 있다고 보고되고 있다. 그러나 아직까지 우리나라 소아청소년에게 적용 가능한 고도 비만 체질량지수(BMI) 진단기준은 제시되지 않았다. 방법:2007년 소아⋅청소년 표준 성장도표를 이용하여다음 두 가지 고도 비만 진단기준을 구하였다; 1) BMI 진단기준 1: 성별 연령별 BMI 95백분위수의 120% 이상 또는BMI 30 kg/m 2 이상, 2) BMI 진단기준 2: 성별 연령별 BMI 99백분위수 이상. 국민건강영양조사 제5기(2010-2012)에참여한 소아청소년을 대상으로 본 연구에서 제시한 진단기준과 성별 신장별 표준체중에 대한 상대체중을 기준으로고도 비만 유병률을 구하였다. 결과: 소아청소년 고도 비만에 대한 BMI 진단기준 1과BMI 진단기준 2의 성별 연령별 BMI 절단값을 제시하였다. 우리나라 소아청소년의 고도 비만 유병률은 BMI 진단기준1에 의해서는 1.6% (남아 1.8%, 여아 1.3%)였으며, BMI 진단기준 2에 의해서는 1.7% (남아 1.4%, 여아 2.1%)였다. 표준체중에 대한 상대체중을 기준에 의한 고도 비만 유병률은 0.8% (남아 0.9%, 여아 0.8%)로 BMI 진단기준 1과 BMI 진단기준 2에 의한 유병률보다 남녀 모두에서 더 낮았다. 결론: 본 연구에서 제안한 소아청소년 고도 비만 진단기준은 국내 정부기관과 관련 학계에서 공식적인 고도 비만진단기준을 권고하기 전까지 우리나라 소아청소년을 대상으로 한 역학조사와 학술 연구에 활용할 수 있을 것이다.
Abstract
Backgroud:Although the obesity prevalence in Korean children and adolescents has stabilized since early 2000s, it is noted that severe obesity is increasing. However, no diagnostic criteria to define severe obesity in Korean children and adolescents has not been recommended yet. Methods:We established two kinds of diagnostic criteria of severe obesity with the 2007 Korean National Growth Charts; 1) body mass index (BMI) criteria 1: BMI≥120% of 95th percentile or 30 kg/m 2 , whichever is lower, 2) BMI criteria 2: ≥99th percentile. We examined the prevalence of severe obesity among children and adolescents who participated in the Fifth Korea National Health and Nutrition Examination Survey, 2010-2012 based on BMI criteria 1 and 2 as well as percentage-weight-for-height (PWH). Results:We presented the age- and sex-specific BMI cut-offs of BMI criteria 1 and 2. The prevalence of severe obesity was 1.6% and 1.7% based on BMI criteria 1 and 2, respectively. The prevalence of severe obesity based on PWH was 0.8%, which was lower than the prevalences based on BMI criteria 1 and 2. Conclusions:Our diagnostic criteria of severe obesity can be utilized in the epidemiological surveys and researches before the recommendation of the formal diagnostic criteria
