한국인 본태성혈소판혈증 환자에서 JAK2V617F 유전자 변이

초록

배경 : 본태성혈소판혈증은 다능성 조혈모세포의 형질전환에의해 발생하나 정확한 분자유전학적 기전은 확실치 않다. 최근 만성골수증식성질환에서 티로신키나제 유전자 변이가 보고되며 분자유전학적 기전으로 대두되고 있다. 이에 저자들은 한국인 본태성혈소판혈증 환자와 반응성혈소판증가증 환자에서 세포질 티로신키나제인 JAK2의 유전자 변이를 조사하였다.방법 : 본태성혈소판혈증 환자 24명의 골수흡인액과 반응성혈소판증가증 환자 36명의 말초혈액에서 대립유전자특이중합효소연쇄반응(allele-specific PCR)으로JAK2 유전자 변이를 관찰하였다.결과 : 본태성혈소판혈증 환자 24명 중 1명 (46%)에서 JAK2유전자 변이 양성소견을 보였으며 반응성혈소판증가증 환자 36명모두 음성이었다. JAK2 유전자 변이 양성군이 고령과 높은 평균백혈구수를 보이는 경향은 있으나 통계적 유의성은 없었다(P=0.172, P=0.094). 그 외 성별, 혈소판수, 헤모글로빈, 비장종대,세포충실도 증가와 섬유증 및 혈관합병증은 두 군간에 유의한 상관관계는 없었다.
