로타바이러스 항원 검출을 위한 라텍스응집법, 면역크로마토그래피법 및 효소면역법의 비교

배경 : 로타바이러스는 소아 장염의 가장 흔한 원인이며, 주로겨울철에 발생한다. 로타바이러스 위장관염은 심한 탈수 및 합병증으로 환자의 생명을 위협할 수 있으므로 보다 신속하고 정확한진단방법이 필요하다. 본 연구에서는 로타바이러스 장염 진단을위해 세 가지 면역크로마토그래피법과 기존의 효소면역법 및 라텍스응집법에 대해 각 민감도와 특이도를 평가하였다.방법 : 이대목동병원 진단검사의학과에 의뢰된 로타바이러스위장관염 환아의 대변 95건을 대상으로 Slidex latex (bioMerieuxVitek, France) 방법 및 세 가지 면역크로마토그래피법; Dip-stick ROTA (Eiken, Japan), SAS Rota Test (SA Scientif-ic, Inc., USA), ASAN Easy Test Rota strip ( 아산제약, 한국)을 시행하였고, 효소면역법으로는 VIDAS Rotavirus (bioMerieuxVitek)를 사용하였다. 위의 검사 결과에서 하나라도 차이를 보이는 경우에는 역전사중합효소연쇄반응검사법 및 임상소견으로 확인하였다.결과 : 총 95건의 검사 중 진양성 56건(58.9%), 진음성 39건(41.1%)이었으며, 검사 결과가 일치하는 경우가 82건(86.3%),일치하지 않는 경우가 13건이었다. 각 검사의 민감도와 특이도는라텍스응집법이 85.7%, 100%, Dipstick ROTA와 SAS Rota는100%, 95%, ASAN Easy Test Rota strip은 86.7%, 87.5%이로타바이러스 항원 검출법 비교 441었고, 효소면역법은 98.1%, 97.3%이었다.
