조현병 환자에서 Paliperidone Extended Release의 효능, 안전성 및 내약성 : 48주 개방형, 전향적 연구의 24주 중간 분석

초록

최근 비정형 항정신병약물이 조현병의 주요 치료제로 확립되었으나, 각 약물의 약물학적 특성 혹은 제형의 차이와 환자개개인의 유전적 특성 등 여러 요인에 의해 치료 반응 및 부작용이 상당히 다르게 나타나므로 효과가 불충분하거나 재발하는 경우가 있다.1) 또한 비정형 항정신병약물과 관련된 대사성부작용, 추체외로계 증상 등의 부작용은 치료 순응도 저하 및증상 재발을 유발하기도 한다.2) Clinical Antipsychotic Trials of Intervention Effectiveness(이하 CATIE) study에서는 조현병 환자 1493명 중 74%가 효과부족 및 부작용 발생으로 18개월 내에 약물을 중단하였다고 보고하였다.3) 현재 조현병의표준치료지침에서는 기존 항정신병약물이 불충분한 효능, 부작용 발생 및 낮은 순응도를 보일 때 다른 항정신병약물로의교체를 권장하고 있다.4) 한국형 조현병 약물치료지침에서도환자의 양성 증상에 대한 효과가 부족하거나 일차적 음성 증상이 지속되는 경우, 혹은 부작용이 해소되지 않을 때 항정신병약물의 교체를 권유하고 있다.5) 따라서 약물 교체시 증상호전뿐만 아니라 부작용 감소 및 약물 순응도 증가를 기대할수 있는 새로운 약물학적 특성을 가진 항정신병약물에 대한요구가 점차 증가하고 있다. Paliperidone extended-release tablet(paliperidone ER)은 risperidone의 대사물로 개발된 비정형 항정신병약물로서osmotic controlled-release oral delivery system(이하 OROS)을 통해 약물 성분이 체내에서 24시간 동안 일정하게 유리되어 혈중 농도의 변화가 최소화되는 장점을 가지고 있다.6) 기존의 6주간 진행된 이중 맹검 위약 대조 연구 및 메타분석에서paliperidone ER은 급성 조현병 치료에 효과적이고 내약성이우수한 것으로 알려졌다.7-9) 유럽에서 진행된 연구8-12)에 따르면 paliperidone ER은 우수한 효능과 내약성을 가지고 있으며,치료 효과가 불충분한 경우 기존의 약물에서 paliperidone ER의 교체가 효과적이었다고 보고하였다. 그러나 인종과 생활방식에 따른 집단의 다양성이 약물의 치료 반응 및 부작용발생에 영향을 미친다는 점을 고려하면13) 아시아에서 paliperidone ER의 효능과 안전성에 대한 추가적인 자료가 필요한 상태이다. 본 연구는 불충분한 치료 반응, 부작용 혹은 낮은 순응도로 기존 비정형 항정신병약물의 교체가 필요한 조현병 환자를 대상으로 paliperidone ER(3~12 mg daily)의 효능, 안전성, 치료 반응과 관련된 요인을 분석하고자 하였다. 본 연구에서는 총 48주의 연구 기간 중 24주까지의 자료를 통해 중간분석을 수행하였다.

Abstract

ObjectivesZZWe investigated the tolerability, safety, and treatment response to flexible-dose paliperidone ER in patients with non-acute schizophrenia in whom previous antipsychotic drugs were ineffective. MethodsZZThis 24-week interim analysis of the 48-week multicenter, prospective, open-label study assessed effectiveness using the Positive and Negative Syndrome Scale (PANSS), Clinical Global Impression-Schizophrenia-Severity (CGI-SCH-S) Scale, Personal and Social Performance (PSP) and Drug Attitude Inventory (DAI). Safety and tolerability were assessed using the Drug-Induced Extrapyramidal Symptoms Scale (DIEPSS) and Liverpool University Neuroleptic Side Effect Rating Scale (LUNSERS). ResultsZZEffectiveness was assessed in 169 patients. Significant improvement in the PANSS total score was observed by week-1 and continued until week-24. The response rate was 33%. The CGI-SCH-S and PSP total scores significantly improved during 24 weeks ; however, no change occurred in the total DAI.
