장기요양보호 재가서비스 이용 노인의 인지기능 손상이 가족 돌봄제공자의 우울에 미치는 영향: 사회적 지지와 자아통제감의 매개효과 검증

초록

본 연구의 목적은 스트레스 과정 이론의 관점에서 장기요양보호 노인의 인지기능 손상, 가족 돌봄제공자에 대한 사회적 지지, 자아통제감, 우울 간의 관계를 검증함으로써 돌봄 가족의 정신건강 증진에 기여할 수 있는 이론적･정책적 시사점을 도출하는 데 있다. 이와 같은 연구목적을 달성하기 위해 이 네 변수들 간에 부분매개 관계가 존재하는 것을 가정하는 연구모형을 설정하였으며, G 광역시에 소재한 30개소의 재가장기요양기관을 이용하는 노인들을 돌보는 가족 돌봄제공자들로부터 수거된 설문지 279부를 대상으로 구조방정식모형 등의 방법을 적용하여 연구가설을 검증하였다. 주요 연구결과는 다음과 같다. 첫째, 노인의 인지기능 손상은 가족 돌봄제공자의 우울을 유의하게 높이는 것으로 확인되었다. 둘째, 노인의 인지기능 손상이 가족 돌봄제공자에 대한 사회적 지지를 매개로 가족 돌봄제공자의 우울에 미치는 간접효과는 유의하지 않았다. 셋째, 노인의 인지기능 손상이 가족 돌봄제공자의 자아통제감을 매개로 가족 돌봄제공자의 우울에 미치는 간접효과는 유의하였다. 끝으로, 이상의 분석결과를 바탕으로 가족 돌봄제공자의 정신건강 증진을 위한 이론적･정책적 함의를 논의하였다.

Abstract

In terms of the stress process theory, the purpose of the present study was to investigate the causal relationships between cognitive impairment of elderly recipients of long-term care, social support for, and depression of family caregivers caring for the elderly in need of long-term care. Based on theoretical backgrounds, a partial mediation research model was proposed. A total of 279 family caregivers from 30 facilities in G Metropolitan City were included in the study and structural equation modeling was used. The primary results were as follows. First, cognitive impairment of older adults was positively associated with family caregivers’ depression. Second, a partial mediation effect of social support between cognitive impairment and depression was not significant. Third, cognitive impairment of older adults was indirectly related to family caregivers’ depression through mastery. Finally, implications of these findings and recommendations for policy and practice were also discussed.
