Abstract

(Background and Purpose) Ostomates, individuals who have undergone colostomy or urostomy surgery, face significant challenges in daily restroom use owing to their unique sanitary and physical needs. Despite the increasing installation of ostomate-friendly facilities in public restrooms, many users have reported difficulties related to operability, hygiene, and psychological discomfort, such as a lack of privacy. Given the diversity in users’ physical conditions and usage contexts, there is a clear need for design strategies grounded in real user experiences. This study aims to develop design guidelines for ostomate-supportive facilities based on a literature review and user studies and to validate the effectiveness of these guidelines through prototype development and usability evaluation. (Method) The research began with a comprehensive literature review and observational studies of individuals with prior experience using ostomate facilities. User interviews and field observations were conducted to identify pain points and behavioral patterns during the use of each facility element. Based on these findings, four core design directions were established: user-centered interface, integrated layout, privacy-enhancing spatial design, and flexible solutions accommodating diverse physical conditions. These directions were translated into a set of Physical and Graphical User Interface (PUI/GUI) design guidelines. A prototype was developed according to these guidelines, and a usability evaluation was conducted with six users using a think-aloud protocol and task-based scenario. The evaluation criteria included effectiveness (task success rate), operability, and safety, which were assessed using a five-point Likert scale. (Results) The usability test demonstrated a general improvement in task success and user satisfaction compared with existing facilities. The key findings included the need to adjust the position and structure of specific elements, such as toilet bowl size, mirror visibility, and button accessibility. Additionally, an expert checklist validation was conducted to assess the clarity, completeness, and feasibility of the guidelines. From this, ten items were confirmed as essential elements (e.g., toilet, lever, shower, mirror, and dispenser), while two were categorized as recommended features (e.g., adjustable device and other auxiliary equipment). The feedback also emphasized the importance of providing design standards in measurable units and considering modular layout strategies for consistent implementation in diverse public settings. (Conclusions) This study proposes practical and user-validated design guidelines for ostomate-supportive restroom facilities. These results underscore the value of user-centered and context-responsive design approaches in public sanitary environments. The findings are expected to contribute to the standardization of ostomate facility design and serve as foundational data for future policy-making and public restroom planning.
