욕창 예방을 위한 케어시트 PUP의 적용 효과 분석 –중환자실 중심으로-

본 연구는 내외과계 중환자실에 입원한 환자를 대상으로 욕창 예방을 위한 케어시트 PUP의 적용 효과를 검증하기 위한 비동등성 대조군 전후 실험연구이다. 2014년 3월 17일부터 9월 16일까지 서울시에 소재하는 E 종합병원의 내외과계 중환자실에서 입원치료 받은 환자를 대상으로 실험군 120명과 대조군 120명을 선정하였다. 실험군에게 케어시트 PUP을 적용하였으며, 대조군에게는 기존의 병원 욕창 간호를 적용하였다. 자료분석을 위하여 SPSS/WIN 20.0 프로그램을 활용하여 t-test와 χ2-test를 시행하였다. 연구결과 케어시트 PUP을 적용한 군은 적용하지 않는 군보다 욕창 발생률이 감소하였고(p=.000), 욕창 단계가 낮음이 확인되었다(p=.000). 결론적으로 중환자실 입원 환자들의 욕창 예방을 위해 케어시트 PUP의 효과가 검증되었다. 그러나 이 결과를 일반화 하기에는 제한점이 있다. 본 연구를 바탕으로 중환자실 환자의 욕창 예방과 관리를 위하여 욕창 예방 드레싱 제품에 따른 비용효과에 대한 추후 연구를 제언하는 바이다.
Abstract
This study aimed to examine the effects of pressure ulcer prevention using Care Sheet Pup focusing on Intensive Care Unit patients. The study is non-equivalent control group pre and-post test design. This study was carried out from 17th March to 16th September in 2014. There were 240 participant who were recruited from the intensive care unit in E general hospital in Korea. 120 patients in the Intensive Care Unit were assigned to an experimental group and 120 patients were assigned to a control group. The experimental group received the Care Sheet Pup and general care of pressure ulcer was given to the control group. The data was analyzed using t-test and chi-square test with SPSS Statistics Win 20.0 program. The incidence of pressure ulcers in the experimental group was lower than the control group(p=.000). There was a lower pressure ulcer stage in the experimental group than the control group(p=.000). To summarize, the results of this study suggest that the Care Sheet Pup dressing is effective in preventing pressure ulcer in Intensive Care Unit patients. However, this is limited to generalize the findings. We propose further research on the cost-effectiveness based on the type of the pressure ulcer dressing in order to prevent and manage pressure ulcer for Intensive Care Unit patients.
