Autoimmune Inner Ear Disease (AIED) : Clinical Point of View

초록
많은 동물실험, 임상 실험에도 불구하고 AIED의 치료는 여전히 high-dose corticosteroid이다. 연구와 치료에진보는 AIED의 발생 기전이 확실히 규명되지 않았고 확실하고 객관적인 검사 방법이 없음으로 보다 나은 새로운 치료법의 개발을 어렵게 하고 있다. 특히 여러 가지 원인들이 난청과 같은 동일한 증상을 유발하고 스테로이드에 반응을 하는 질환, 대표적인 것이 AIED의 경우 임상적으로 연구하는데 상당한 어려움이 있다. 향후 질환의 원인 혹은 원인 항원의 규명, 전신질환에속발한 secondary AIED와 다른 부위의 침범 없이 내이에만 발생한 primary AIED의 발생기전의 차이점, 객관적인 진단 검사법의 개발 및 고막 내 국소치료법에서 치료를 위한 최적의 용량 및 횟수 등 에 많은 진보가 있을것으로 생각되며 병이 진행이 수개월에 양측성으로 난청이 초래 되어 인공와우 이식까지 시행 받게 되는 경우가 있지만 조기에 발견하여 적절한 치료를 하면 청력이회복되는 드문 reversible 한 질환임을 상기하고 임상의사로서 조기 진단과 신속하고 적절한 치료법의 개발에주목할 필요가 있다.
