전이개 재건술(total auricular reconstruction)은 이개의 형태가 기형이 심하거나 거의 존재하지 않는 경우에 연골, 피부 및 보형물을 사용하여 전체 귀의 형태를 다시 재건하여 주는 것을 의미한다. 전이개 재건술이 필요한 기형은 Weerda의 분류1) 중 Grade 2~3의 기형이며, 이 중 Grade 3의 기형은 남아있는 이개의 형태에 따라 이개강(cavum concha), 이수(ear lobule) 및 대주(antitragus)의 형태가 남아 있는 이개강형(concha type)과 이수와 비슷한 피부의 일부만 남아 있는 이수형(lobule type) 및 귀의 형태가 전혀 남아있지 않은 무이증 등으로 구별할 수 있다(Fig. 1).2) 이들 기형은 대개 이개뿐만 아니라 외이도, 고막, 이소골 및 중이강의 기형이 동반되므로, 이개의 재건 시에는 외이의 형태뿐만이 아니라, 추후에 재건해 주어야 하는 외이도의 위치 및 청력까지 종합적인 면을 재건 시작 전에 고려해야 한다. 1870년 Szyanowski가 처음으로 전이개 재건술을 보고한 후에3) 재건방법은 점차로 발전하여 Tanzer,4) Brent,5) Nagata,6) Weerda,7) Firmin 등8)에 의해 그 방법이 확립되었다. 본문에서는 전이개 재건술의 재료부터 방법까지 전체적인 면을 검토해 보고자 하며, 주로 필자가 사용하는 Weerda와 Siegert에 의해 제안된 변형된 Nagata 술(modified Nagata’s techni-que)2)에 대하여 주로 이야기하고자 한다.
