초록

Facial nerve schwannomas are uncommon, benign tumors that originate from the facial nerve, posing complex challenges in both diagnosis and treatment. In this case, a 42-year-old female presented with a painless, recently enlarging mass below her right ear without facial nerve paralysis. Radiological examinations initially suggested a mass in the parotid gland, and fine-needle aspiration biopsy provided inconclusive results. However, concerns regarding facial nerve involvement during surgery raised suspicions of malignancy, which were later confirmed through an intraoperative frozen biopsy as a schwannoma. Therefore, we decided to preserve facial nerve and left a portion of the tumor. Subsequently, the patient's facial nerve paralysis improved from House-Brackmann grade 5 immediately after surgery to grade 3 at four months postoperatively. Clinicians should always maintain an index of suspicion for the small but possible likelihood of encountering tumors such as facial schwannomas. In this report, we aim to discuss key aspects of this intraparotid facial nerve schwannoma case, with a particular focus on the diagnostic and treatment processes.

Abstract

Subsequently, the patient's facial nerve paralysis improved from House-Brackmann grade 5 immediately after surgery to grade 3 at four months postoperatively. Clinicians should always maintain an index of suspicion for the small but possible likelihood of encountering tumors such as facial schwannomas. In this report, we aim to discuss key aspects of this intraparotid facial nerve schwannoma case, with a particular focus on the diagnostic and treatment processes.
