구강검진 및 필요치과진료 수진의 결정요인: 경제적 접근성과 지리적 접근성 간의 상호작용효과 분석

구강건강 수명을 늘리고 구강건강 관련 삶의 질의 향상을 도모하기 위해서는 구강검진과 필요치과진료를 적시에 받는 것이 중요하다. 본 연구는 질병관리본부의『2009 지역사회건강조사』와 국민건강보험공단의『2009 지역의료이용통계』 자료를 이용하여 ‘구강검진’ 및 ‘필요치과진료’의 수진 여부에 미치는 영향요인을 분석하였다. 분석 결과 거주지역 치과의사 밀도와 가구소득이 높을수록 ‘구강검진 수검’ 확률은 높았고 ‘필요치과진료 미수진’ 확률은 낮았다. 거주지역 치과의사 밀도와 가구소득의 상호작용효과는 구강검진 수검 및 필요치과진료 미수진 여부에 각각 유의한 영향을 주었다. 이 외에도 인구사회학적 수준, 신체적 건강상태, 구강 건강상태 등의 다양한 요인이 구강검진 수검 및 필요치과진료 미수진 여부에 영향을 주는 것이 확인되었다. 구강보건의료의 접근성을 증진하고자 하는 기존의 정책 대부분이 지불 측면을 고려한 ‘경제적 접근성’에 집중되고 있는 바, 구강 의료인력과 의료시설에 대한 가용성을 고려한 ‘지리적 접근성’에 대한 고려가 함께 필요하다 할 것이다.
Abstract
In order to extend the life of oral health and improve quality of life in relation to oral health, it is important to receive dental screening regularly and receive treatment timely. The purpose of this study is to identify determinants of dental screening and unmet dental needs using Community Health Survey and Regional statistics of National Health Insurance. The analysis shows that the higher the density of dental practitioners and household incomes were, the higher the usage of dental screening was, and the lower the probability of experiencing unmet dental needs was. The interaction effect between the density of dental practitioners and household incomes proved statistically significant. In addition, demographic features, physical health and oral health were found to be related with whether to receive dental screening and experience unmet dental needs. While most of the policies to improve accessibility to oral health services have so far focused on economic affordability, i.e. the ability to pay, this study finds that geographical accessibility is required as well in improving accessibility to oral health services.
