치과의료이용 불평등에 영향을 미치는 결정요인 분해

초록

본 논문은 치과의료이용의 불평등 정도를 확인하고, 그 불평등에 영향을 미치는 요인을 파악하여 정책적 판단 근거를 제공하고자 2008～2011년도 한국의료패널자료를 이용하여 실증적 분석을수행하였다. 단 순 치과의료이용 방 문 횟수가 아 닌 방문당 지출한 의료비를 반영하여 의료이용의자원사용도를 고려한 치과의료이용 집중지수를 산출했다. 또한 성-연령, 구강질환 관련 만성질환보유여부, 섭식장애, EQ-5D 건강가중치 등 의 변수를 포함하여 수평적 형평성 지수를 산출하였다 . 분석결과 치과의료이용의 집중지수 및 수평적 형평성 지수는 모 두 고소득층에게 유리한 불평등을 보였으며, 의료 필요도를 반영할 경우 불평등 정도는 약간 감소되는 것으로 나타났다. 치과의료이용의 불평등에 영향을 미치는 요 인 중에서 소 득 , 연 령 , 교육수준, 섭식장애, 민간의료보험 가입형태 등 이 높 은 기여도를 보였다. 소득은 단일변수로는 가 장 큰 기여도를 보였으며, 인구학적 요인 보다는 사회경제적 요인이 더 큰 영향을 미치는 것으로 나타났다. 사회경제적 지위가낮 은 집단에 대 한 치과의료의 접근성을 높이기 위 한 보 다 실효성 있 는 정 책 개발이 요구된다.

Abstract

The aim of this study was to examine the degree of inequalities in dental care utilization and determinant factors for the inequity in a effort to suggest the health policy intervention. Using data from 2008-2011 Korean Health Panel data, concentration indexes for dental care utilization were calculated by reflecting medical expenditure per visit and horizontal inequity indexes also adjusted by needs for dental care(gender, age, eating disorder and EQ-5D weighting value of health). As a result, the concentration indexes and the horizontal inequity indexes were both unequal in favor of the higher income groups. The level of the inequity became lower when needs for dental care were reflected. Resulting from income, age, eating disorder and the form of subscription to private health insurance made a great contribution to inequality in dental utilization, which is decomposed the determinant factors to that. Income was the most influential single variable and socioeconomic factors were more influential than demographic factors. Therefore more efficient policy implication is required to improve the access of the groups with lower socioeconomic status to dental care.
