암 사망자의 호스피스·완화의료 이용과 관련 요인

초록

호스피스·완화의료 확산 정책 마련에 기초자료를 제공하고자, 암 사망자를 대상으로 호스피스·완화의료 이용 특성과 관련 요인을 살펴보았다. 국민건강보험 데이터베이스를 이용하여 2019년 1년간 사망한 암 환자의 의료이용을 분석한 결과, 사망 전 1년 이내 한번이라도 호스피스·완화의료를 이용한 자는 22.1%였으며, 미이용자는 77.9%이었다. 호스피스·완화의료 이용자와 미이용자는 성, 연령, 의료보장 및 건강보험 분위, 거주 시도, 암 종류에 따라 그 특성을 달리하였으며, 남성, 80세 이상, 의료급여 대상자, 충남 및 충북 거주자, 암 종류가 “행동약식불명 또는 미상 신생물”, “림프, 조혈, 관련조직 악성신생물”일 때 호스피스·완화의료 이용이 취약하였다. 또한 호스피스·완화의료 이용자는 미이용자에 비해 기관내 삽관술 또는 인공호흡 등 적극적 시술처치 시행률이 1.2%로 미이용자 15.8%에 비해 낮았다. 호스피스·완화의료 서비스 이용은 입원형 서비스만 단독 이용한 경우가 67.0%로 가장 많았고, 다음이 입원형과 자문형 병행 이용 11.2%, 자문형 단독 이용 11.1% 순이었다. 말기환자들이 편안하고 품위 있는 죽음을 맞이하기 위해서 호스피스·완화의료의 확산이 필요하다. 또한 호스피스·완화의료는 성, 연령, 지불능력, 거주지, 진단범주, 기타 환경적 요소에 무관하게 공평하고, 지체 없이 제공될 필요성이 있다. 그동안 정책적 노력들에 더해 호스피스·완화의료가 더욱 확산되기 위해서 호스피스·완화의료 이용자 특성을 기반으로 한 구체적이고 세밀한 정책 개발이 필요한 시점이다.

Abstract

This study aims to identify Hospice and Palliative care(HPC) utilization and associated factors of terminal cancer patients in Korea, and to further provide an empirical evidence of HPC expansion policy. To evaluate HPC by terminal cancer patients in the last year of life, data were used 2019 deaths from the National Health Insurance Database. Among all terminal cancer patients, 22.1% received a HPC, and associated with gender, age, SES, geographic residence, and type of cancer. Findings revealed that male, individuals with low socio-economic status, residing in rural areas, and ‘malignant neoplasms of lymphoid, hematopoietic and related tissue’, ‘neoplasms of uncertain or unknown behavior’ were less likely to use HPC. HPC patients were significantly related to lower rate of aggressive care. By the provision type, the HPC is classified into hospitalization, consultation, and home-based treatment. Among all HPC patients, 67.0% received a hospitalization only, 11.2% received a hospitalization and consultation, 11.1% received a consultation only, in order. HPC is needed to ensure dignity and quality of life in all individuals, regardless different circumstances. This study provides a reference for decision-making in the future expand of HPC.
