중·고령 입원환자의 퇴원 후 서비스 이용 장소 결정 요인: 청구자료를 활용한 공간 분석 및 다수준 분석

Determinants of discharge places for middle-aged and elderly patients: spatial and multi-level analysis using claims data

초록

급성기 이후 환자들의 재활과 회복, 지역사회 복귀를 위한 보건의료서비스의 제공은 국내외를 막론하고 중요한 아젠다이다. 특히 탈시설화 및 지역사회중심 서비스의 제공이 국내외에서 주목받고 있음에 따라, 본 연구는 중·고령 입원환자의 퇴원 후 지역사회 복귀, 즉 ‘가정으로의 퇴원’에 초점을 맞춰 지역의 변이 및 결정 요인을 파악하고자 한다. 국민건강보험공단에서 제공하는 맞춤형데이터와 통계청에서 제공하는 시군구 단위의 지역자료를 자료원으로 활용하였고, 공간 분석 및 다수준 분석 방법을 적용하였다. 분석 결과, 입원환자의 후급성기 보건의료서비스 이용 장소(가정, 요양병원, 급성기병원)는 지역적으로 변이를 갖고, 지역의 보건의료자원 수준에 영향을 받으며, 공간효과 또한 유의하게 나타남을 확인하였다. 2010, 2018년 자료를 활용하여 가정으로의 퇴원 결정 요인을 살펴보는 다수준 분석에서는 공통적으로 여성, 고령, 가구원 수가 적고, 동반질환이 많은 환자들이 가정으로의 퇴원 가능성이 낮았다. 그러나 기관수준 변수의 영향은 연도에 따른 결과가 상이하여 2010년 결과와 달리 2018년엔 대학병원 및 대규모 병원 입원환자가 가정으로의 퇴원 가능성이 낮았다. 이는 그동안 추진된 보건의료 정책의 변화에 따른 것으로 해석되며, 본 연구의 결과는 향후 이와 같은 변화가 재정 및 건강에 미치는 장기적 효과를 평가할 수 있는 후속 연구의 필요성을 시사한다. 또한 공간 분석에서 확인된 지역의 영향은 다수준 분석에서는 유의하지 않아 환자의 서비스 이용 장소는 개인 및 기관의 특성에 의해 주로 결정됨이 확인되었고, 이는 지역 간 형평적인 자원의 제공과 더불어 개인의 취약성과 서비스 제공기관 간 서비스 격차 또한 고려되어야 함을 보여준다.

Abstract

The provision of health care services for the rehabilitation and recovery of patients after the acute phase and their return to the community is an important agenda worldwide. In particular, as deinstitutionalization and community-centered services are attracting attention, this study aims to identify regional variations and determinants of discharge places (homes, acute hospitals, and LTCH) of middle-aged and elderly patients in Korea, focusing on ‘discharge to the home'. Claims data from National Health Insurance Service and regional data from Statistics Korea was used, and spatial analysis and multi-level analysis methods were applied. As a result of the analysis, it was found that the discharge site has regional variations, is affected by local health care resources, and has a significant spatial effect. In the multi-level analysis using 2010, 2018 data, women, older people, patients with fewer households, and patients with more commodities were less likely to be discharged to their homes. The impact of institutional-level variables differed by years, with patients admitted to teaching hospitals and large hospitals being less likely to be discharged home in the 2018. This could be the result of the changes in Korea's policies related to post-acute health care services. The results of this study suggest the need for further studies to assess the long-term financial and health effects of these complex changes. In addition, the regional impact identified in the spatial analysis was not significant in the multi-level analysis, confirming that the discharge place is largely determined by individual and institutional characteristics, indicating that individual vulnerabilities and service gaps between service providers should also be considered.
