정신장애인 가족의 외상 후 성장 영향요인

Influencing Factors on Posttraumatic Growth among Family of Adult with Mental Disorder

초록

본 연구의 목적은 정신장애인 가족들이 경험하는 외상사건특성과 가족들의 외상 후 성장(Posttraumatic Growth: PTG)에 영향을 미치는 요인들을 확인하기 위한 것이다. 이를 위해 서울경기, 충청, 호남지역의 정신보건관련 기관을 이용하는 264명의 정신장애인가족들을 대상으로 위계적 회귀분석을 사용하여 분석하였다. 연구결과 첫째, 정신장애인의 주 진단명은 조현병이 가장 높게 나타났고, 가족원의 관계는 부모가 많았으며, 정신장애인의 평균유병기간은 11.41년으로 높게 나타났다. 둘째, 인구사회학적 특성에 따른 변수들의 차이를 살펴본 결과, 남성, 대학원이상의 학력을 가진 가족원들의 외상 후 성장수준이 유의미하게 높게 나타났으며, 60대에서 돌봄 부담 정도가 가장 높았고, 특히 가족들 중 정신장애인 배우자의 돌봄 부담 정도가 가장 높은 것으로 나타났다. 정신질환진단명, 유병기간, 가족수입에 따른 외상 후 성장의 유의미한 차이가 나타나지 않았으나 조현병 가족들의 돌봄부담 수준이 가장 높은 것으로 나타났다. 셋째, 정신장애인 돌봄 만족경험이 높을수록, 사회적 지지가 높을수록, 외상지각이 높을수록, 돌봄 부담 수준이 낮을수록 정신장애인 가족의 외상 후 성장에 유의미한 영향을 미치는 것으로 나타났다. 이러한 연구결과에 기반하여 정신장애인 가족의 성장을 지원하고 증진시키기 위한 사회복지 개입 방안들을 제언하였다.

Abstract

Families of people with mental disorder have experienced a greater burden and psychosocial trauma. However family members of people with mental disorder have been understudied regarding on their posttraumatic growth(PTG). This study was conducted to examine the influencing factors of posttraumatic growth(PTG) among family members of adult with mental disorder. This study was conducted on 264 family members with questions on caregiving experience, trauma sensitivity, social support, socio-mographic factors, PTG. Collected data were analyzed by t-test, ANOVA, step wise regression. The main results were as follows: First, family with schizophrenia and spouse showed more higher care burden and over 60 year old, male, higher education showed high PTG. Second, positive caregiving was the most significant predictor of PTG. Third, positive caregiving, social support, higher trauma sensitivity, lower caregiving burden were significantly impacted on the PTG. Based on these findings, practical strategies of intervention for social worker were suggested to support family with mental disorder and to improve their quality of life.
