한국인 폐쇄성수면무호흡증후군 환자의 임상특성과 한국판 Epworth Sleepiness Scale의 임상적 유용성

초록

목적 : 국내에서 폐쇄성수면무호흡증후군(obstructive sleep apnea syndrome, OSAS) 환자의 임상특성과 위험인자를 밝혀내고 한국판 Epworth Sleepiness Scale의 임상적 유용성 및 폐쇄성수면무호흡증후군의 중증도와의 연관성을 알아보기 위해 본 연구를 시행하였다.방법 : 2002년 1월 3일부터 2004년 12월 31일까지 영남대학교 신경정신과 수면클리닉을 방문한 환자중 무호흡-저호흡지수가 5 이상이며 주기적 사지운동 증후군, 하지불안 증후군, 수면제 복용, 기면병, 알코올 남용한 경우를 제외한 158명을 대상으로 시행하였다. 검사 일주일 전부터 수면일지를 완성하여 평균수면시간이 최소 6시간 이상인 경우 본 실험에 포함하였으며 수면다원검사를 시행하기 전 Epworth Sleepiness Scale을 작성하였다.결과 : 폐쇄성수면무호흡증후군의 중증도와 유의하게 연관된 인자는 desaturation index와 최저산소포화도였다. 체질량지수, 평균산소포화도, 초기산소포화도 및 한국판 Epworth Sleepiness Scale과 폐쇄성수면무호흡증후군의 중증도와의 사이에서는 유의한 연관성을 보이지 않았다. 결론 : 폐쇄성수면무호흡증후군에서 Epworth Sleepiness Scale의 임상적 유용성에 관한 연구는 추후 다양한 방법으로 이루어져야할 것이다.

Abstract

Objectives : The study has been performed in order to find out the clinical characteristics and risk factors of patients with obstructive sleep apnea syndrome in Korea, the clinical usefulness of Epworth Sleepiness Scale (in Korean edition) and the correlation between Epworth Sleepiness Scale and the severity of obstructive sleep apnea syndrome. Methods : Among the patients who have visited sleep clinic, department of Neuropsychiatry from Yeongnam University from Jan. 3, 2002 to Dec. 31, 2004, 158 patients whose apnea-hypopnea index was more than 5 and who had no cases of periodic limb movement syndrome, restless leg syndrome, intake of sleeping pills, narcolepsy and the abuse of alcohol have been the subjects of study. Sleep diary was completed one week before the investigation and that the average sleeping time is at least more than 6 hours is included in this experiment and Epworth Sleepiness Scale was completed before the performance of polysomnography. Results : The factors related significantly to the severity of obstructive sleep apnea syndrome were desaturation index and lowest oxygen saturation. There was no significant correlation between body mass index, mean oxygen saturation, baseline oxygen saturation and Epworth Sleepiness Scale (in Korean edition) and the severity of obstructive sleep apnea syndrome. Conclusions : In this study, desaturation index and lowest oxygen saturation reflect the severity of obstructive sleep apnea syndrome. The further study on the clinical usefulness of Epworth Sleepiness Scale in obstructive sleep apnea syndrome should be performed in various ways in the future.
